About the Role

We are seeking a highly skilled Registered Nurse/Registered Psychiatric Nurse to join our Community Mental Health team at Fraser Health Authority. As a key member of our team, you will provide treatment and care management services to individuals with mental illness and concurrent disorders.

Key Responsibilities
  • Coordinate care for clients by collaborating with the interprofessional care team, performing intake functions, conducting mental health assessments, and developing comprehensive treatment plans.
  • Provide direct nursing care to clients, including reviewing client/family history, conducting physical/psychosocial assessments, and implementing treatment modalities such as ADLs, wound care, and medication administration.
  • Work from a strengths-based model, using motivational interviewing and engagement techniques to develop and implement individualized care plans.
  • Establish and maintain therapeutic relationships with clients and their informal support network, engaging stakeholders in treatment planning and evaluation.
  • Provide treatment services, including crisis intervention, individual counseling, and education, according to various theoretical models.
  • Initiate and participate in case conferences/meetings with clients, family, service providers, and members of the interprofessional team to ensure a coordinated approach to treatment.
Requirements
  • Graduation from an approved School of Nursing or Psychiatric Nursing.
  • Two years of recent related experience providing treatment services to individuals with mental health and/or concurrent disorders.
  • Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Valid Class 5 BC Driver's License and access to a personal vehicle with appropriate insurance coverage.
