About the Role

We are seeking a highly skilled and compassionate Family Medicine Specialist to join our team at the WELL Health Clinic Network. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and promoting health and wellness in our community.

Key Responsibilities
  • Patient Assessment and Care
    • Conduct thorough medical histories and physical examinations to identify patients' health needs
    • Develop and implement individualized treatment plans to address patients' health concerns
    • Order and interpret diagnostic tests, such as laboratory tests and imaging studies, to inform treatment decisions
  • Health Promotion and Education
    • Provide patients with education and guidance on healthy lifestyle choices, disease prevention, and management
    • Develop and implement health promotion programs to improve patient outcomes
  • Collaboration and Communication
    • Work collaboratively with other healthcare professionals to coordinate patient care
    • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
  • Administrative and Clinical Responsibilities
    • Manage patient records and maintain accurate and up-to-date documentation
    • Participate in quality improvement initiatives to enhance patient care and safety
Requirements
  • Education and Certification
    • Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree
    • College of Family Physicians Certification (CCFP) or equivalent
  • Skills and Experience
    • Proven experience in family medicine or a related field
    • Excellent communication and interpersonal skills
    • Ability to work effectively in a fast-paced environment
