Job Overview

As a vital component of our multidisciplinary team, the Nurse Practitioner will leverage clinical proficiency and comprehensive understanding of pediatric, adolescent, and adult care to strategize and assess patient management. The ideal candidate will exhibit leadership qualities and foster excellence in nursing by delivering expertise in clinical practice, consultation, education, and research within the realm of addiction. The Nurse Practitioner will incorporate research findings into practice to uphold best practice standards and enhance clinical initiatives.

Key Responsibilities

This role's primary duties include identifying, screening, assessing, consulting, and providing clinical interventions for individuals experiencing Mental Health and Substance Use Disorders, along with their families. Responsibilities encompass specialized assessment, coordination, and treatment for those affected by these disorders. The clinician will offer counseling, education, and support services to individuals, families, and the community, assisting clients in understanding and addressing issues related to Mental Health and Addiction.

The clinician is expected to collaborate effectively with an inter-professional team within the Mental Health Program and establish partnerships with relevant community organizations. This position is integral to the addiction services program within Mental Health.

Required Qualifications

Halton Healthcare is in search of professionals who exemplify leadership in their field. The successful candidate will be a natural collaborator with strong communication skills, demonstrating a patient-centered, compassionate, and respectful approach while being fiscally responsible. The ideal candidate will possess:

  • A Master's Degree in Nursing or a recognized equivalent is preferred.
  • Current registration in the Extended Class with the College of Nurses of Ontario, ideally with a Primary Health Care specialty certificate.
  • A Nurse Practitioner's Certificate from an accredited program.
  • Current membership in a professional association such as CNA, RNAO, or NPAO is preferred.
  • Completion of the Opioid Dependence Treatment Certificate or a commitment to obtain it within the next year.
  • A minimum of three years' experience in the field of substance use disorders/addiction.
  • At least five years of experience in Mental Health, including both inpatient and community-based services.
  • Comprehensive knowledge of substance use and associated disorders, their causes and effects, and evidence-based treatment models applicable to diverse populations.
  • Expertise in conducting bio-psychosocial assessments and strong formulation skills to develop person-centered, strength-based strategies that reflect evidence-based practices and clinical experience.
  • Experience in motivational interviewing and trauma-informed care.
  • Ability to provide opioid addiction treatment using a harm reduction approach.
  • Capacity to deliver an integrated model of care that connects clients to primary care and/or enhances existing relationships with primary care providers to meet client needs.
  • Knowledge of individual, family, group, and crisis counseling methods.
  • Proven skills in working with families of individuals with mental health and substance use disorders.
  • Established comfort in working within a harm-reduction framework.
  • Documented experience collaborating with inter-professional teams in a fast-paced hospital environment.
  • A solid understanding of diverse cultures and the principles of cultural safety.
  • Experience in tailoring treatment services and interventions to the unique circumstances of clients and their families.
  • Proficiency in MS Office applications and related electronic health care platforms.
  • A satisfactory employment and attendance record.
  • Experience in psychoeducation and psychotherapy is preferred.

Work Schedule: Monday to Friday - hour days; hours and days may vary based on operational needs.

Compensation: Competitive hourly rate.
