Key Responsibilities
This role encompasses the identification, screening, assessment, consultation, and provision of clinical interventions for individuals grappling with Mental Health and Substance Use Disorders, along with their families. Responsibilities include:
- Conducting specialized assessments, coordination, and treatment for individuals facing Mental Health and Substance Use challenges.
- Offering counseling, education, and support services to individuals, families, and the broader community, aiding clients in understanding and addressing mental health and addiction-related issues.
- Collaborating effectively with a multidisciplinary team within the Mental Health Program and fostering partnerships with relevant community organizations.
- Participating in the addiction services program within Mental Health.
Qualifications
Halton Healthcare is in search of individuals who exemplify leadership in their field. The successful candidate must be a collaborative team player with strong communication skills, demonstrating a patient-centered, compassionate, and respectful approach. The following qualifications are required:
- Master's Degree in Nursing or a recognized equivalent is preferred.
- Current registration in the Extended Class with the College of Nurses of Ontario is mandatory, ideally with a Primary Health Care specialty certificate.
- Nurse Practitioner's Certificate from an accredited program.
- Current membership in a professional association such as CNA, RNAO, or NPAO is preferred.
- Completion of the Opioid Dependence Treatment Certificate or a commitment to obtain it within the next year.
- A minimum of three years' experience in the field of substance use disorders/addiction.
- At least five years of experience in Mental Health, including both inpatient and community-based services.
- In-depth knowledge of substance use and associated disorders, including causes, effects, and evidence-based treatment models applicable to diverse populations.
- Expertise in conducting bio-psychosocial assessments and strong formulation skills to develop person-centered, strength-based strategies reflecting evidence-based practices.
- Experience in motivational interviewing and trauma-informed care.
- Providing opioid addiction treatment with a harm reduction approach.
- Implementing an integrated care model that connects clients to primary care and/or enhances existing relationships with primary care providers.
- Knowledge of individual, family, group, and crisis counseling methodologies.
- Demonstrated ability to work with families of individuals with mental health and substance use disorders.
- Proven experience with concurrent mental health and substance use disorders.
- Established comfort working within a harm-reduction framework.
- Documented experience collaborating with inter-professional teams in a fast-paced hospital environment.
- Understanding of diverse cultures and principles of cultural safety.
- Experience in tailoring treatment services and interventions to meet the unique circumstances of clients and their families.
- Ability to work independently.
- Proficiency in MS Office applications and related electronic health care platforms.
- Satisfactory employment and attendance record required.
- Experience in providing psychoeducation and psychotherapy is preferred.
Work Schedule: Monday to Friday - hour days; hours and days may vary based on operational needs.
Compensation: Competitive hourly rate.
