Job Summary

Halton Healthcare is seeking a skilled Registered Practical Nurse to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and working collaboratively with other healthcare professionals to achieve optimal patient outcomes.

Key Responsibilities
  • Provide direct patient care to acute medical and surgical populations, including assessment, planning, implementation, and evaluation of patient care.
  • Administer medications and treatments as prescribed by physicians and other healthcare professionals.
  • Conduct physical assessments and develop individualized care plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Participate in ongoing education and professional development to stay current with best practices and advancements in the field.
Requirements
  • Current registration with the College of Nurses of Ontario.
  • Current Heart & Stroke Approved BLS Provider Card upon hire.
  • Experience working with and advocating for the elderly.
  • Medication administration certification and experience.
  • Physical assessment and wound care skills.
  • Enteral feeding experience and maintenance of IV lines.
  • Experience with electronic documentation.
  • Satisfactory employment and attendance record.
Working Conditions

Hours: Monday - Sunday, Days and Evenings. Hours are subject to change based on operational needs.

Salary: $ per hour - $ per hour.
