Key Responsibilities- Provide specialized assessment, coordination, and treatment for individuals with Mental Health & Substance Use Disorders and their families.
- Develop and implement individualized treatment plans that address the unique needs of clients.
- Collaborate with inter-professional teams to provide comprehensive care and support services to clients and their families.
- Provide counseling, education, and support services to individuals, families, and the community.
- Establish and maintain partnerships with related community organizations to enhance service delivery and promote community engagement.
- Master's Degree in Nursing or recognized equivalent.
- Current registration in the Extended Class with the College of Nurses of Ontario, preferably with the Primary Health Care specialty certificate.
- Nurse Practitioner's Certificate from a recognized program.
- Current membership in professional associations such as CNA, RNAO, or NPAO.
- Completion of the Opioid Dependence Treatment Certificate or a willingness to obtain within the next 1 year.
- Minimum three years' experience working in the field of substance use disorders/addiction.
- Minimum five years of experience working in the field of Mental Health, including both inpatient and community-based services.
- Thorough knowledge of substance use and associated disorders, causes, and effects.
- Expertise in conducting bio-psychosocial assessments and strong formulation skills.
- Experience in motivational interviewing and trauma-informed care.
- Ability to work independently and as part of an inter-professional team.
- Proficient with MS Office applications and related electronic healthcare platforms.
