Hawthorn Woods Community is seeking a highly skilled and compassionate Nurse Practitioner to join our team. As a critical member of our healthcare team, you will play a vital role in providing high-quality patient care and promoting a positive patient experience.
Key Responsibilities:
- Provide primary care to residents in accordance with established policies and procedures, as well as within the scope of practice.
- Collaborate with physicians to provide follow-up care and support, as needed.
- Provide nursing consultation to residents, families, healthcare professionals, and other stakeholders.
- Participate in the identification, analysis, and interpretation of trends in patient care outcomes and professional nursing practice issues.
- Respond promptly to resident concerns and implement action plans to prevent reoccurrence.
Requirements:
- Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class or RN (EC).
- Demonstrated experience in gerontology.
- Advanced knowledge and decision-making skills in assessment, diagnosis, and healthcare management.
- Knowledge of primary healthcare issues in long-term care.
- Strong interpersonal and leadership skills.
- Ability to pass a Vulnerable Sector Check.
Preferred Qualifications:
- Certification from the Canadian Nurses Association.
