Patient Care Facilitator – Lifemark Health Group

The Patient Care Facilitator plays a vital role in establishing a positive connection between the clinic and its clientele, which includes patients, healthcare professionals, funding bodies, and referral partners. This is accomplished by delivering exceptional administrative assistance and managing clinic operations with a warm, enthusiastic, and professional demeanor towards all patients, team members, and external stakeholders.

The Patient Care Facilitator is accountable for overseeing all elements of patient onboarding, data management, handling phone inquiries, making outbound calls, processing faxes and emails, as well as managing invoicing and payment collections.

Key Responsibilities

Manage incoming calls, welcome patients and visitors upon their arrival, and ensure outstanding customer service for all inquiries or issues Facilitate a thorough and precise collection of information during the patient's initial visit to the clinic and throughout their treatment journey Assist patients in completing necessary documentation at the clinic to minimize errors and ensure accurate information Organize the scheduling of patient appointments and maintain comprehensive medical records Collect payments at the time of service delivery Monitor coverage for all funding sources to guarantee approval Conduct data entry and billing processes Coordinate the handling of courier packages (incoming and outgoing) and manage incoming and outgoing emails, mail, and faxes Provide administrative support to the Patient Care Facilitator Manager and Clinic Director

Required Qualifications & Core Competencies

High School Diploma, G.E.D. or equivalent Exceptional customer service abilities and telephone communication skills Strong organizational skills with the capacity to prioritize and manage multiple tasks Excellent verbal and written communication skills, along with strong interpersonal abilities Proficient computer skills and data entry capabilities Adaptable and flexible in response to change Experience in a Physiotherapy clinic is advantageous
