Extendicare Canada Inc. is seeking a highly skilled and dedicated Nurse Practitioner to join our team in providing exceptional primary health care services to residents in our long-term care facilities.
Key Responsibilities- Deliver Comprehensive Health Services: Provide primary health care to residents, including the management of acute and chronic medical conditions, health promotion, and disease prevention.
- Lead Clinical Care Teams: Provide clinical leadership to long-term care team members, focusing on resident care needs, including admission/discharge planning, palliative care, and pain management.
- Conduct Health Assessments: Perform comprehensive health assessments, using and adapting assessment tools and techniques based on resident needs, and complete health histories.
- Order and Interpret Diagnostic Investigations: Order diagnostic investigations and interpret results using evidence-based clinical reasoning.
- Develop Person-Centered Plans of Care: Collaborate with residents and family members to develop person-centered plans of care, synthesizing health information using critical inquiry and clinical reasoning.
- Communicate Health Information: Communicate health assessment findings and diagnoses to residents, including outcomes and prognosis, using knowledge of pathophysiology, psychopathology, epidemiology, and behavioral sciences.
- Implement Interventions: Select and implement appropriate interventions to restore or maintain resident functional, physiological, and mental stability.
- Promote Safe Resident Care: Mitigate harm and address immediate risks for residents and others affected by adverse events and near misses.
- Collaborate with Discharge Planning Services: Liaise with discharge planning services for hospitalized residents to ensure a smooth transition back to the long-term care home.
- Participate in Quality Improvement Initiatives: Collaborate with the leadership team on quality improvement and research initiatives, and participate in professional advisory committees.
- Registration: College of Nurses of Ontario registration in the Extended Class.
- Education: Baccalaureate in Nursing (BScN), Masters Degree in Nursing (MScN or MN) an asset.
- Professional Memberships: Membership with Nurse Practitioners' Association of Ontario (NPAO) (preferred).
- Liability Insurance: Has acquired professional liability insurance (with either RNAO or other liability insurance).
