About the Role

CarePartners is seeking a skilled Physiotherapist to join our dynamic Rehabilitation Team. As a key member of our team, you will provide high-quality patient care in the community and in patients' homes.

Key Responsibilities
  • Conduct professional assessments of patients in the community and in their homes
  • Develop and maintain effective care plans in collaboration with the care team
  • Work with patients to set and achieve time-specific, measurable goals and outcomes
  • Provide one-on-one therapeutic support to clients and promote their independence in the home and community
Requirements
  • Master's Degree in Physiotherapy from a recognized university
  • Registration with the College of Physiotherapists of Ontario
  • Proven computer skills and strong assessment skills
  • Excellent English verbal and written communication skills
  • Reliable vehicle for regular travel
