COMPENSATION: $53,630 – $65,589 Annually Prorated

CONTRACT DURATION: 14 Months

Black Youth Can is a dedicated organization committed to empowering youth facing various challenges, including mental health issues, substance use, and socio-economic barriers. We are seeking a Case Manager to join our team on a full-time contract basis.

Position Overview:

As a Case Manager, you will operate both independently and collaboratively within a team to create a robust support network for clients experiencing mental health challenges, addiction issues, and physical health concerns. Your role will focus on empowering clients, enhancing their independence, dignity, and self-worth in a secure environment. You will provide direct assistance in daily living, crisis intervention, and facilitate connections to essential community services tailored to individual needs.

Key Responsibilities:

  • Formulate personalized support and recovery plans by identifying client priorities and goals in collaboration with clients and other professionals.
  • Encourage personal empowerment through coaching and mentoring, assisting clients in daily living activities and life skills development, including hygiene, medication management, and budgeting.
  • Establish and manage a comprehensive support network that includes social, medical, and community resources, developing coordinated care plans as necessary.
  • Provide crisis intervention and assist clients in creating effective crisis management plans.
  • Represent the program and the organization professionally, promoting services to clients, families, and external agencies.
  • Facilitate conflict resolution among residents and provide follow-up to ensure resolution.
  • Engage in planning and executing events to address clients' social needs and reduce isolation.
  • Maintain accurate records and statistics, ensuring compliance with established procedures.
  • Collaborate with team members to enhance program delivery, identify service gaps, and participate in meetings and planning initiatives.

Qualifications:

Essential:

  • A relevant degree in human services with a strong understanding of recovery and psychosocial rehabilitation principles, along with a minimum of three years of experience with the target population.
  • Familiarity with concurrent disorders and harm reduction strategies.
  • Insight into the challenges faced by adults with mental health issues, addiction, and homelessness.
  • Ability to foster therapeutic relationships and negotiate client goals effectively.
  • Knowledge of local community resources and professional services available to clients.
  • Experience working collaboratively within a professional team.
  • Valid driver's license and comfort with client transportation.
  • Strong interpersonal, communication, and counseling skills.
  • Proficient in crisis intervention, advocacy, and problem-solving.
  • Computer literacy for maintaining records and statistical data.
  • Excellent organizational and time management skills.

Preferred:

  • Physical capability to perform demanding tasks.
  • Willingness to travel frequently to various program sites.
  • Understanding of cultural competency and anti-oppression frameworks.
  • Experience reflecting the diverse backgrounds of the client population is an asset.
