Position Overview:
As a Community Support Worker, you will play a vital role within a vibrant team committed to delivering care and assistance to individuals living in supported residences within their community. Your responsibilities will include motivating and enabling the individuals we serve to lead self-sufficient lives while maintaining high standards of care, health, and safety. Your natural ability to support others and work effectively with a multidisciplinary team will be essential to the overall success of our mission.
Key Responsibilities:
- Honor the rights and preferences of supported individuals and advocate on their behalf when needed
- Engage meaningfully with the individuals we assist
- Encourage individuals to take an active role in their community
- Contribute to the creation and execution of Personal Service Plans
- Build positive relationships with families, professionals, colleagues, and community members
- Support daily living activities and assist with medication management
- Possess knowledge of behavior management techniques, mental health considerations, and documentation practices
- Ensure cleanliness and organization in individuals' residences, including meal preparation and light housekeeping duties
Qualifications:
- High school diploma
- Community Support Worker Diploma or equivalent qualification
- At least 2 years of experience in community social services, healthcare, or caregiving
- Experience in specialized areas such as Dysphagia, mobility aids, and medical procedures
- Valid driver's license with a clean driving record
- Fully vaccinated against COVID-19
- Clear criminal background check
- Basic computer proficiency
- Eligibility to work in Canada
