TRAINING OPPORTUNITY

In the event that there are not enough qualified applicants for this position, Vancouver Coastal Health (VCH) may provide an 8-week training program. Interested candidates must submit a cover letter and resume. Those who accept this training opportunity commit to working with VCH in the relevant nursing area for a minimum of 18 months following the completion of the training. This ensures that candidates can effectively consolidate and apply the skills acquired in the specialty area that VCH has supported and funded. Should a candidate leave the nursing area for which they were hired within 18 months, VCH reserves the right to recover the training costs.

Salary

Job Overview

Vancouver Coastal Health (VCH) is seeking a Community Health Nurse.

We are looking for Registered Nurses to become part of the Home Care Nursing team.

Responsibilities of the Community Health Nurse include:

Providing care for clients with acute, chronic, and palliative care requirements. Collaborating with clients and their families to set safe, realistic, and achievable goals by assessing, planning, coordinating, organizing, implementing, evaluating, and transitioning nursing and other services. Evaluating service eligibility and facilitating the development, implementation, and adjustment of care plans. Delivering direct care and managing care to help clients navigate through various community services. Consulting with other system partners and integrating current evidence into practice.

About the Location

The community is located on the traditional territories of the shíshálh (Sechelt) Nation and is approximately 50 km northwest of a major city. It is accessible via a ferry followed by a short drive. This picturesque seaside community is renowned for its natural beauty and is a favored spot for outdoor activities such as sightseeing, boating, diving, camping, and mountain biking. The area features several parks, numerous dining options, local shops, boutiques, galleries, and a golf course. Qualifications

Education & Experience

Current registration as a Registered Nurse with the relevant nursing regulatory body. A minimum of one (1) year of recent clinical experience in a relevant field, particularly with complex clients/patients facing chronic health conditions and/or mobility challenges, or an equivalent combination of education, training, and experience. A valid driver's license is required as local travel may necessitate the use of a vehicle or other accepted transportation methods.

Knowledge & Skills

Proven knowledge and skills in community health nursing theory and practice within a client and family-centered care model. Ability to provide trauma-informed, harm reduction, and culturally safe care through comprehensive client assessments and interventions. Strong critical thinking skills to assess safety and risk in various home environments and effectively integrate and evaluate data from multiple sources. Problem-solving skills to manage psychosocial and behavioral issues, employing effective conflict resolution techniques. Demonstrated independence in managing and prioritizing client needs while promoting cooperation and innovative care planning. Resilience in adapting to new or unexpected situations while maintaining a focus on client-centered care. Collaborative skills to work effectively within an interdisciplinary team to assess, plan, and monitor a variety of health challenges. Proficiency in documenting timely and relevant information in electronic medical records. Commitment to maintaining client confidentiality and privacy. Effective communication skills to build rapport and maintain professional boundaries with clients, families, and healthcare professionals. Ability to teach and facilitate client engagement in care planning and self-management. Familiarity with provincial regulations and guidelines related to home and community care. Knowledge of resources available for client and family support. Ability to plan, organize, and prioritize work effectively. Physical capability to perform the duties required for the position.
