Job Title: Social Worker

Job Responsibilities:

As a Social Worker, you will be responsible for:

  • Providing direct psychosocial care, counseling, system navigation, and advocacy for clients and their families
  • Collaborating with other healthcare providers and the multidisciplinary team to develop and implement care plans
  • Facilitating client self-management and providing education and linkage to relevant specialized complex care resources
Scheduling Information:

Shift times are from 08:00 to 16:00 hours. We offer a flexible scheduling system to ensure that you have a healthy work-life balance.
