Job Summary:

As a Clinical Social Worker with Interior Health Authority, you will provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families. Working collaboratively with other healthcare providers and the multidisciplinary team, you will provide direct psychosocial care, counseling, system navigation, and advocacy, education, and linkage to relevant specialized complex care resources while facilitating client self-management.

Key Responsibilities:

Provide social work services to maintain or improve the health and social well-being and functioning of the identified population.

Collaborate with other healthcare providers and the multidisciplinary team to provide comprehensive care to clients and their families.

Develop and implement care plans that address the unique needs of each client.

Provide education and support to clients and their families to promote healthy behaviors and self-management skills.

Requirements:

Bachelor's degree in Social Work or a related field.

Registration with the British Columbia College of Social Workers.

Experience working with vulnerable populations and providing psychosocial care.

Strong communication and interpersonal skills.
