Job Summary

WoodGreen is seeking a skilled Case Manager to join our Supportive Housing team. As a Case Manager, you will play a critical role in providing comprehensive case management services to individuals in our supportive housing program.

Key Responsibilities
  • Provide assessment, personal goal setting, and short/long-term planning to individuals in the program.
  • Develop and implement case plans that address the unique needs of each individual.
  • Provide informal counseling, follow-up supports, crisis intervention, and service coordination.
  • Link clients to various community resources and facilitate referrals to other community support services.
  • Ensure the coordination of services and a continuum of care.
  • Liaise, consult, and collaborate with other program area staff and service providers.
  • Monitor housing challenges and ability of each resident to adhere to policies and protocols.
  • Complete statistics and reports, and maintain all records regarding program/activity enrollment.
Requirements
  • Bachelor's degree in Social Work, Social Service Work Diploma, or a related discipline.
  • 3+ years related experience in mental health and harm reduction, housing help, and working with those who have experienced homelessness.
  • Ability to work effectively in a team and independently.
  • Strong housing help skills and ability to support individuals to incorporate a harm reduction strategy in obtaining and maintaining their housing.
  • Ability to exercise good judgment, flexibility, creativity, and sensitivity to changing needs.
  • Ability to minimize barriers through service delivery for low-income individuals with mental health and substance use issues.
