Key Responsibilities- Provide care coordination and support for patients, families, and healthcare staff for patients seeking medical assistance in dying.
- Lead and apply specialized theory and knowledge relating to mental health, community services, and palliative care to promote safe, ethical, and quality care.
- Act as a clinical resource, providing consultation support to patients, families, and healthcare teams.
- Develop and deliver educational programs and materials for staff, patients, and families.
- Participate in direct patient care when required.
The VCH Assisted Dying Program supports Medical Assistance in Dying (MAiD) in VCH, ensuring equitable access to MAiD as a patient-centred and patient-driven care option.
Qualifications- Current full registration with the British Columbia College of Social Workers (BCCSW) is required.
- Masters' of Social Work degree from an accredited School of Social Work, supplemented with three (3) years' recent, related acute, facility or community experience supporting mental health, vulnerable populations, and end-of-life care.
- Valid BC Driver's License as local area travel requires the use of a vehicle.
