Job Summary:
We are seeking a highly skilled Social Worker to join our interdisciplinary team at Providence Health Care. As a Social Worker, you will provide therapeutic clinical counselling services to patients, families, and groups, and conduct comprehensive biopsychosocial assessments to identify patient needs and develop effective treatment plans.
Key Responsibilities:- Provide therapeutic clinical counselling services to patients, families, and groups, including exploring client concerns, facilitating change in coping styles, and coordinating services with interdisciplinary treatment teams.
- Conduct comprehensive biopsychosocial assessments, including interviewing patients and families, gathering social data, and interpreting data to inform treatment plans.
- Maintain patient records, including documenting assessments, charting patient information, and preparing progress notes and reports.
- Contribute to discharge planning, including assessing patient needs, determining eligibility for services, and coordinating with community partners to develop and implement discharge plans.
- Provide consultative and direct services to patients, families, staff, and community health professionals regarding community resources and access to these resources.
- Participate in clinical care team conferences, including patient reviews, admissions, and discharges.
- Support patients, families, and teams in understanding legislative processes and criteria related to healthcare consent, medical assistance in dying, and adult guardianship.
- Develop, coordinate, and deliver in-services, presentations, and training related to designated programs for staff, in collaboration with other members of the interdisciplinary team.
- Participate in local committees, including program evaluation, identifying opportunities for change, and assessing available resources.
- Facilitate and participate in group meetings, including facilitating group, providing clinical counseling services, and providing therapeutic and/or psycho-social education as required.
- Maintain broad knowledge of developments in the Social Work profession, including reviewing literature, consulting with clinical and community experts, and participating in professional development activities.
- Masters in Social Work from an accredited university.
- One year of recent related experience in a designated program area, or an equivalent combination of education, training, and experience.
- Current full registration with the British Columbia College of Social Workers.
