Patient Access Manager

Fraser Health is seeking a highly skilled and experienced professional to fill the role of Manager, Patient Access & Community Transition. This position plays a critical role in ensuring the smooth transition of patients between hospitals and community care settings, while also providing operational oversight and management of patient access and flow.
Key Responsibilities- Provide leadership and management of patient access and flow, ensuring quality patient care and effective patient outcomes.
- Collaborate with department and clinical support areas to identify barriers to patient flow and develop strategies to address them.
- Develop and implement seamless transition points between hospital and community care settings.
- Monitor and evaluate access and flow performance, utilizing MOH and FH criteria/indicators.
- Establish and maintain effective working relationships with medical staff, other departments/programs, and external stakeholders.
- Completion of an undergraduate degree in health sciences or related field.
- Seven years' recent related experience in patient access and flow, discharge planning, and Alternate Level of Care within a complex healthcare environment.
- Two years in a leadership role.
- Demonstrates the leadership practices of the Fraser Health Leadership Framework of Clear, Caring, and Courageous.
- Ability to develop and maintain effective working relationships with others.
- Excellent oral, written, and presentation skills.
- Demonstrated knowledge of budget planning and preparation principles, concepts, and methods.
