We are seeking a highly skilled and compassionate Reminiscence/Memory Care Coordinator to join our team at Sunrise Senior Living. As a key member of our care team, you will be responsible for providing exceptional care and support to our residents with dementia and their families.
Key Responsibilities- Develop and implement individualized care plans that promote the physical, emotional, and social well-being of our residents.
- Lead and coordinate reminiscence programs and activities that stimulate the minds and engage the hearts of our residents.
- Collaborate with our care team to ensure seamless delivery of care services and support to our residents and their families.
- Conduct regular assessments and evaluations to identify the changing needs of our residents and make recommendations for adjustments to their care plans.
- Develop and maintain relationships with local Alzheimer's Association chapters and other community organizations to stay up-to-date on best practices and resources for dementia care.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellence in care delivery.
- College degree in a related field (e.g., gerontology, social work, nursing).
- At least one year of experience in dementia care, preferably in a senior living setting.
- Strong leadership and communication skills, with the ability to work effectively with diverse stakeholders.
- Ability to work independently and as part of a team, with a focus on providing exceptional customer service.
- Knowledge of dementia care principles and practices, including the Resident-Centered model.
- Ability to learn and adapt to new technologies and systems.
