Job Summary

Fraser Health is seeking a skilled Licensed Practical Nurse (LPN) to join our multidisciplinary mental health services team as an Outreach Nurse. In this role, you will work collaboratively with RNs and RPNs to provide high-quality care to clients in their homes and in the community.

Key Responsibilities
  • Administer medications in accordance with pharmaceutical and organizational policies and procedures.
  • Assess and monitor clients' physical, psychological, socio-cultural, and spiritual needs.
  • Provide a supportive environment for clients and contribute to the development and modification of their care plans.
  • Plan, organize, and establish client care plan priorities when providing care.
  • Respond to unexpected events and changing client needs within scope of practice and competency guidelines.
  • Provide nursing care to clients according to identified policies and procedures, technology, and Fraser Health standards.
  • Evaluate outcomes, record observations, and report problems and/or changes to designated staff.
  • Document and update relevant care and procedures information to ensure patient safety.
  • Assist clients with activities of daily living, such as serving meals and feeding, client transports, bathing, dressing, skin care, toileting needs, and oversee client exercise routines as applicable.
  • Participate in quality improvement and program activities to maintain a consistent level of care and ensure interdisciplinary team members work collaboratively.
  • Maintain a safe working environment for patients and others by adhering to established safety policies and reporting unsafe conditions and/or malfunctioning equipment.
  • Assist in the orientation and education of new staff, students, and other members of the interdisciplinary team in collaboration with the Manager or designate.
Qualifications
  • Graduation from a recognized program of Practical Nursing or an equivalent combination of education, training, and experience.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Current First Aid, CPR, and Food Safe certificates.
  • Completion of or eligible to complete MHSU Competency Education.
  • Valid BC Driver's License and access to a personal vehicle with appropriate insurance coverage.

  • Ability to operate related equipment.
  • Ability to organize and prioritize.
  • Ability to establish and maintain rapport with patients.
