About the Role

The Licensed Practical Nurse (LPN) is a key member of our healthcare team, responsible for coordinating care delivery and providing individualized resident-centered care. This role requires a strong commitment to teamwork, collaboration, and patient-centered care.

Responsibilities:
  • Assess residents' needs and develop individualized care plans.
  • Ensure residents' rights are observed and maintain a holistic approach to wellness.
  • Liaise with physicians, community agencies, families, and other healthcare professionals to ensure quality care.
  • Assess and treat wounds, and provide education to care staff on skin breakdown prevention.
  • Administer medications and treatments within scope of practice.
  • Monitor and report changes in residents' physical and psychological conditions.
  • Participate in interdisciplinary care conferences and care plan reviews.
  • Support administrative requirements, including admissions and confidentiality.
  • Provide coaching and direction to care staff, and participate in orientation and education.
  • Maintain up-to-date knowledge of community policies and procedures.
Qualifications and Skills:
  • Graduate of an approved Practical Nursing program and current licensure with the British Columbia College of Nurses and Midwives.
  • Valid Occupational First Aid Level 1 certification.
  • Successful annual Resident Assessment Instrument (RAI) competency testing.
  • Previous experience in long-term care an asset.
  • Collaborative and team working skills.
  • Ability to read, write, and speak English proficiently.
  • Computer skills, including basic navigation of Word, Excel, and Outlook.
