Responsibilities:
  • Provide patient care and support to clients in their homes, focusing on their physical, emotional, and social well-being.
  • Develop and implement individualized care plans, taking into account the client's unique needs and preferences.
  • Collaborate with healthcare professionals, family members, and caregivers to ensure comprehensive care and support.
  • Administer medications, perform medical procedures, and provide wound care as required.
  • Document patient progress, maintain accurate records, and communicate effectively with the healthcare team.
Requirements:
  • Bachelor's degree in Nursing or equivalent.
  • Current registration with the provincial or territorial nursing regulatory body.
  • Experience in home care or a related field is an asset.
  • CPR and First Aid certification.
  • Valid driver's licence and reliable transportation.
