About the Role:

We are seeking a skilled Registered Practical Nurse (RPN) to join our team in providing high-quality community care to patients in various settings. As a member of our team, you will have the opportunity to work with patients of all ages, providing scheduled visits and delivering care within your scope of practice.

Key Responsibilities:

  • Participate in scheduled visits to assess patient needs and provide care
  • Use assessment tools and techniques to identify individual patient needs
  • Develop and implement care plans, monitoring and adjusting as required
  • Engage patients in a collaborative approach, building rapport and trust
  • Maintain accurate and up-to-date patient records

Requirements:

  • Registration with the CNO as a Registered Practical Nurse in good standing
  • Experience in delivering community care is an asset
  • Reliability, flexibility, and a willingness to travel to patient residences
  • Experience working with vulnerable populations and a passion for making a difference in the community
