Ottawa, Ontario, Canada Sherbourne Health Full time
Internal/External Posting

Job Title: Nurse Practitioner

Job ID #: IHS FTNP-25-0225

Department: Integrated Health Services

Reports To: Manager, Primary Care

Salary Range: $60.84 – $67.82 per hour

Status: Full-Time, Permanent

Hours: 37.5 hours per week

Vacancy: 1

Who We Are:

Sherbourne is a leading provider of quality health care and transformative support to people who face social, economic, and other systemic barriers. Our mission is to be a dynamic provider of integrated health services, community programs, and capacity-building initiatives that enable people and diverse communities to achieve wellness.

Sherbourne provides innovative health care and social services to the diverse urban population of southeast Toronto. While our doors are open to everyone, we focus on people experiencing homelessness or who are under-housed; 2SLGBTQ+, and newcomers to Canada. We are funded by the Ministry of Health, Ontario Health, Ministry of Community, Children and Social Services, the City of Toronto, and many generous donors and foundations.

About the Role:

Working closely with other NPs at Sherbourne Health, the successful Nurse Practitioner (NP) has the opportunity to support multiple programs within the Integrated Health Services portfolio. This includes providing comprehensive primary health care for individuals in an ambulatory setting as part of our Family Health Team (FHT), supporting healthcare needs of homeless and underhoused clients in our short-stay Acute Respite Care (ARC) program, and providing harm reduction services through prescribing as part of our Safer Opioid Supply (SOS) Program. The NP works collaboratively with an interprofessional team and external health care providers to support clients with chronic disease management, health promotion, illness prevention, substance use management, and healthcare system navigation.

About the Programs the NP Supports:

ARC: Working closely with other Nurse Practitioners at Sherbourne Health, the NP will also provide support in the delivery of healthcare to our Acute Respite Care (ARC) program. ARC is a 24hr/day 14-bed short-term medical respite program for persons who are homeless or precariously housed who require support in a community setting with nursing and daily living to support recovery from acute medical conditions or exacerbations of chronic diseases to undergo certain treatments (e.g., for cancer). ARC is also funded to support Trans and Non-Binary clients as they recover from transition-related surgeries.

FHT: Sherbourne Health's Family Health Team is an award-winning primary care program providing clinical health services, paired with wrap-around social and community programs, and we support clients in our target communities to achieve their holistic health goals.

Working at Sherbourne

At Sherbourne Health, we care about our staff and recognize that our biggest strength is our people. We have a solid commitment to life-long learning which is demonstrated through our professional development program. We invest in collective work and staff training to improve client care. We supplement this by creating a low-barrier work environment built on equity and respect while working together in caring, cohesive teams and providing opportunities for advancement.

- Integrate a broad knowledge base to perform holistic health assessments to obtain the required information to determine diagnoses, client needs, and associated interventions throughout the client's time at Sherbourne Health.
- Support clients with internal and community referrals as needed.
- Work collaboratively with clients to identify and mitigate health risks, promote understanding of health issues, and support healthy behaviours.
