about the role:
Working closely with other NPs at Sherbourne Health, the successful Nurse Practitioner (NP) has the opportunity to support multiple programs within the Integrated Health Services portfolio. This includes providing comprehensive primary health care for individuals in an ambulatory setting as part of our Family Health Team (FHT), supporting healthcare needs of homeless and underhoused clients in our short-stay Acute Respite Care (ARC) program, and providing harm reduction services through prescribing as part of our Safer Opioid Supply (SOS) Program. The NP works collaboratively with an interprofessional team and external health care providers to support clients with chronic disease management, health promotion, illness prevention, substance use management and healthcare system navigation. The Sherbourne FHT NP understands the impact of the broad determinants of health on people’s ability to attend to their health care needs and provides care within a health promotion framework.
Sherbourne health supports diverse populations including those who are homeless or underhoused, have mental health and substance use issues, are newcomers to Canada and are 2SLGBTQ. The NP will have an in-depth understanding of the health issues that are experienced by these populations and other marginalized persons and will work in partnership with care providers, both internally at Sherbourne and externally, in the community to improve the health of clients accessing our programs.
About the programs the NP supports:
ARC: Working closely with other Nurse Practitioners at Sherbourne Health, the NP will also provide support in the delivery of healthcare to our Acute Respite Care (ARC) program. ARC is a 24hr/day 14-bed short-term, medical respite program for persons who are homeless or precariously housed who require support in a community setting with nursing and daily living to support recovery from acute medical conditions or exacerbations of chronic diseases to undergo certain treatments (e.g. for cancer). ARC is also funded to support Trans and Non-Binary clients as they recover from transition related surgeries.
FHT: Sherbourne Health’s Family Health Team is an award-winning primary care program providing clinical health services, paired with wrap-around social and community programs, and we support clients in our target communities to achieve their holistic health goals.

Responsibilities and Tasks:
- Work closely within a multidisciplinary team to provide primary care throughout the client’s life span.
- Provide health care for clients presenting in Sherbourne programs with substance use issues, mental health issues, acute medical issues, post operative care, injuries, cancer treatment and/or acute exacerbation of chronic issues. This care may be provided independently within the scope of practice outlined by the College of Nurses or in consultation with a primary care physician or other health care providers as appropriate.
- Integrate a broad knowledge base to perform holistic health assessments to obtain the required information to determine diagnoses, client needs and associated interventions, throughout the client’s time at Sherbourne Health.
- Support clients with internal and community referrals as needed.
- Work collaboratively with clients to identify and mitigate health risks, promote understanding of health issues, and support healthy behaviours.
- Offer safe, effective, and ethical prescribing for clients who are treated with controlled substances.
- Collaborate with team members, agency partners, clients, and communities to plan, implement, and evaluate health promotion activities that target specific health care needs identified through community health care needs assessments.
- Perform outreach to marginalized individuals and groups, advocating for clients, and supporting community development.
- Actively participate in quality improvement initiatives and organizational meetings and committees
- Participates and supports the formation of policies and procedures.
- Supervise NP learners to develop and refine competencies needed for safe, ethical, and effective practice
- Provides leadership in clinical and nursing care for Integrated Health Services
To thrive in this role, you’ll need
education/experience:
- Current registration as a Registered Nurse in the Extended Class with the College of Nurses of Ontario (CNO)
- Current Basic Life Support (BLS) certification
- Evidence of completion of CNO-approved controlled substances education for Nurse Practitioners
- Minimum 3-5 years of experience as a Registered Nurse
- Minimum of 2-3 years clinical experience as a Nurse Practitioner (especially in primary care/community settings) an asset
- Experience working with persons marginalized by social location including homelessness, poverty, sexual identity, gender identity, and/or residency status, using trauma-informed, anti-oppressive and harm reduction practices
- Experience working with individuals who face mental health and substance use issues
- Knowledge and understanding of primary prevention and health promotion theory and practice
- Experience and/or strong interest in applying harm reduction principles to reduce negative health consequences associated with drug use among people who use drugs, including opiate agonist therapy (OAT) and safer opioid supply
- Clinical teaching experience an asset
skills and abilities:
- Proven ability to work both independently and within a multidisciplinary team
- Solid interpersonal and crisis intervention skills
- Effective leadership, organizational, critical-thinking, problem-solving, decision-making and communication skills.
- Experience and comfort working with an electronic medical record (EMR)
- Working knowledge of Microsoft Office suite, including Word, PowerPoint, and Microsoft Teams is an asset.
- Demonstrates an understanding of working within an anti-oppression framework
- Working knowledge of acts, legislation, and professional standards that govern nursing practice including the Personal Health Information Protection Act (PHIPA), Regulated Health Professions Act (RHPA), and College of Nurses of Ontario (CNO) Standards and Guidelines
- Ability to maintain professional practice regarding commitments to colleagues and clients, pertinent legislation, evidence-based standards of care and professional standards of practice
- Ability to speak languages in addition to English is an asset

working conditions/environment:
- The NP primarily works onsite at Sherbourne Health (333 Sherbourne Street) with occasional work at other sites in the community when applicable.
- Based on operational requirements, the position may require you to work days, evenings and weekends. The usual hours of work are Monday to Friday, 7.5-hour per shifts.
