Join our team todayHow will you create an impact: In accordance with the established vision and values of the organization, the Regional Cardiovascular Coordinator collaborates with patients as well as physician and administrative leaders within the Interior Health (IH) Cardiac Program and leads the appropriate coordination of patient pathways to ensure timely and appropriate access to care for the assigned program area or clinical services. This includes the development, implementation, and evaluation of the assigned clinical services across IH. The Coordinator works closely with other Cardiovascular Coordinators and service providers across BC and within IH to ensure timely, efficient, and equitable access to services for all IH patients regardless of geographic location.In alignment with IH’s Occupational Health & Safety Program, employees shall adhere to all Occupational Health and Safety policies and procedures at all times and attend all required training. Employees are responsible to report any identified hazards, unsafe conditions or incidents to your manager or supervisor immediately.What will you work on:• Collaborates with Physician Leads, the Program Director, Medical Director(s), and affiliated team members for the purpose of development, implementation, and evaluation of integrated services for the assigned program area or clinical services.• Functions as a partner’s liaison in facilitating communication to affect positive outcomes for patient and family satisfaction.• Develops and implements a case management model for patients that results in increased patient satisfaction, quality care, a decreased length of stay, and decreased utilization of resources.• Participates in collaborative processes with other Cardiovascular Coordinators and Cardiac Services BC in an ongoing evaluation of waitlist management and data entry systems. Develops processes and tools to support revisions of the system as necessary.• Provides recommendations toward the establishment of sustainable systems and processes, as required, to assist referring physicians within the hospital and between hospitals to assist in timely patient access to services. This includes ensuring that referrals are dealt with in a timely manner and include complete demographic, clinical, and contact information as well as relative urgency. Ensures systems are in place to support communication among the healthcare providers involved in the care of patients.• Participates in research to advance the delivery of care for cardiac patients and their families. Collaborates in the development of interdisciplinary standards of care for patients. Participates in the development of systems to facilitate the care and management of patients and families from pre-hospital to community integration and follow-up.• In collaboration with Cardiac Services BC, participates in the development, implementation, and ongoing maintenance of a current, complete, and accurate database for all IH patients awaiting procedures, including tracking patient wait times and stability while awaiting their procedure across IH. Maintains current waiting lists, identifying patients exceeding the recommended maximum waiting time. In conjunction with the BC Cardiac Registry, ensures that statistics for waiting lists and cases concluded are complete and recorded in the database in a timely manner.• Establishes systems to notify physicians of changes in patients’ conditions. Works with physicians to develop options to facilitate patients having procedures completed within recommended waiting time guidelines.• Provides patient- and family-centered support and educational information to patients and families awaiting services by facilitating referrals to appropriate services internally or externally (e.g., counselling, risk factor modification, and cardiac rehabilitation) based on assessment of patients’ needs and service availability across IH. • Provides a key liaison function for external and internal providers and patients. Provides pertinent information to partners using a variety of verbal, written, and electronic communication methods to facilitate decision-making and timely access to care. Communicates and collaborates with Cardiovascular Coordinators in other healthcare institutions in BC and the BC Cardiac Registry.• Identifies and participates in addressing system gaps impacting the quality of patient care. This may include pre-admission patient education, communication regarding waitlist status, ongoing telephone support to waitlisted patients, coordination of in-patient versus out-patient scheduling priorities, coordinating triage rounds to ensure the most urgent cases take priority, and ensuring improvement to waitlist management. • Works closely with Cardiac Services BC to integrate the tracking of quality indicators and outcomes. Prepares and presents monthly statistics to the Cardiac Program team. Leads and/or participates in continuous quality improvement initiatives to optimize program effectiveness and efficiency.• Sets measurable goals and objectives within a team or individual environment, incorporating the vision, values, goals, and corporate plan of the hospital through methods such as updating clinical knowledge and professional development within the area of practice and developing a plan in collaboration with patient care leaders; reviews progress to ensure goals are achieved within established timeframes. • In alignment with IH’s Occupational Health & Safety Program, employees shall adhere to all Occupational Health and Safety policies and procedures at all times and attend all required training. QualficationsEducation, Training and Experience• A Bachelor’s degree in nursing.• Five years of recent (within last two years), related experience in the assigned program area or clinical services in an acute care hospital environment with demonstrated expertise in program development, project coordination, or project management. • Or an equivalent combination of training and experience.• Prior experience in the assigned clinical services is required.• Experience in the use of research/patient registry databases, report writing, and identifying needed measurement indicators.• Current practicing membership with the British Columbia College of Nurses and Midwives (BCCNM).Skills and Abilities• Ability to communicate effectively, both verbally and in writing.• Comprehensive knowledge of healthcare delivery systems.• Demonstrated ability to work independently and in collaboration/consultation with others.• Demonstrated skills in leadership, communication, mediation, and negotiation.• Demonstrated skills in the application of communication and computerized technology to address communication needs and information analysis.• Demonstrated ability to work in a fast-paced environment under pressure and maintain priorities.• Demonstrated ability to think laterally, solve problems, make decisions, and take action in a crisis management environment.• Demonstrated ability to work diplomatically and persuasively to establish and maintain effective working relationships with internal and external partners.• Physical ability to perform the duties of the position.
