SSA is looking for a full-time Team Coordinator to join our wonderful Hospice administrative team in providing overall administrative and scheduling support to the clinical team.

Responsibilities:

1. Schedules hospice aide staff and coordinates cross coverage of hospice aides during absences
2. Administers Durable Medical Equipment (DME)
3. Helps with managing the supply room inventory
4. Prepares EMR Workflow and Reports and assists with IDG meetings
5. Assists with all documents' verifications, transfers, revocations, releases and ensures they are completed, signed and processed within established time frames
6. Scans/uploads forms into the EMR document library
7. Reviews and responds to telephone messages, emails and in-person patient requests for information
8. Answering phones as a member of the call center and helps provide front desk coverage as needed
9. Participates in all required meetings
10. Promptly reports all incidents and/or issues to Nurse Manager or Office Manager as appropriate

Qualifications:

1. High School Diploma or equivalent, associate degree preferred
2. 3-5 years' experience performing related work
3. Demonstrated organizational and time management skills
4. The ability to multitask and work in a fast-paced environment
5. Demonstrated flexibility, adaptability, and outstanding customer service skills
6. Proficient with MS Office 365, Electronic Medical Records and other databases
7. Familiarity and comfort with medical terminology
8. Ability to manage confidential information with sensitivity
9. Capable of communicating professionally with patients, power of attorneys, legal guardians, primary care physicians and other community/medical providers
10. Outstanding phone and in-person customer service skills
11. Able to work in the office Monday through Friday from 8:30am-5pm
