This is a Part-Time position in Burks' Falls, ON area.

Hours of work are within a 2-weekrotationinclusive of weekends and on-call work.

What The Role Involves

- Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with Ontario Health @Home (OH@Home).
- Working with patients (both in-person and virtually) to provide exceptional nursing services
- Providing services one-on-one with patients in a home setting
- Communicating and liaising with patients, family members and others within the interdisciplinary care team
- Performing other clinical duties as required

What You Bring

- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- A strong desire and commitment to making a difference in the lives of our patients
- Exceptional interpersonal skills
- A valid driver's license and vehicle as regular travel is required
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record including 2 step TB test
- To protect our patients, we require a current vulnerable sector check and a clear background check
