**(Full-Time)**

At Community Care Durham, coordinated networks of volunteers and inter-professional staff, working out of integrated community hubs and other locations, strive to enrich the well-being and quality of life for people who choose to live at home. As the largest community support services agency in Durham Region, CCD collaborates with its partners and the broader community to address community issues and support local citizens to achieve their goals to live independently.

Reporting to the Vice President of Client Services, the Director of Personal Support Services is responsible for the leadership, guidance and support to the Personal Support Services Management team, collaborates with community partners to drive strategic initiatives that enhance client care and service delivery to meet organizational goals, and oversees the quality and efficiency of Personal Support Services programs. The Director will collaborate with the CEO, VP and the Senior Management Team in developing and leading strategies to improve the experience and outcomes of care for all Community Care Durham (CCD) clients within the In-Home Supports programs (Assisted Living Services, Supportive Housing, Respite, etc.).

Responsibilities include but are not limited to:

- Collaborates with Management team to streamline and improve processes, problem-solve issues, develop best practices, create synergies and ensure a holistic client experience;
- Leads a team of Program Managers and Supervisors, Care Coordinators, Program Assistants, Personal Support Workers, and Homemakers;
- Monitors MSSA targets, plans and implements practices alongside the Vice President to ensure annual targets are achieved;
- Manages fiscal responsibilities through the development of program plans/target adjustments, budgets, and administration of financial resources within budget;
- Leads program expansion projects (creates, presents, and executes business plans, oversees requests for proposals (RFP), etc.
- Represents Community Care Durham to external stakeholders and various external collaborations (i.e., Lakeridge Health Durham OHT, Home Care Modernization), and fosters collaborative relationships with other community partners (i.e., community support agencies, hospitals, Ontario Health at Home, etc.,)
- Provides leadership and best practice support in managing escalated client and caregiver concerns and Incident Reports regarding client safety issues for efficient quality services;
- Identifies changes to existing policies, processes and guidelines related to service, and ensures changes are effectively deployed and communicated;
- Identifies and manages risk as it relates to vulnerable clients and the local services;
- Provides leadership and support to staff as part of Manager On-Call rotation (weekends and holidays as needed) and communicates incidents/situations with appropriate Manager/staff as needed.

**Qualifications required**:

- Post-secondary education in Healthcare, Social Services or related field
- Minimum five (5) years in a Director or Senior Management role required
- Minimum five (5) years’ experience in a health care or community not for profit setting
- Knowledge and experience using client record management systems, client scheduling systems, medical records and external reporting
- Valid “G” Ontario driver’s license and unlimited access to a safe and reliable vehicle
- Strong critical thinking and time management skills with the ability to work in a fast-paced environment
- Self-directed and highly motivated with excellent interpersonal and communication skills both written and verbal
- Strong proficiency in budget and target management
- Experience creating and utilizing business plans and proposals
- Highly proficient computer skills and experience using Microsoft Office (Word, Excel, Share Point, etc.)
- Is committed to innovation and continuous quality improvement
- Is committed to ongoing professional development with a focus on leadership in the community health and services sector
- Sensitivity to the needs of the frail elderly and persons with disabilities, including challenges in cognition
- Must work in a safe manner and promote the health and safety of all staff, volunteers, clients and the community as outlined within CCD policies

**Compensation**:Starting at $95,000 annually; four (4) weeks’ vacation per year of work (pro-rated to hours worked); 9 paid (including statutory) holidays, 3 float days; health, insurance, and pension benefits when eligible.
- Pay will be commensurate based on education and experience
