Job Summary:
We are seeking a compassionate and experienced Team lead

Key Responsibilities:

- Conduct initial assessments and ongoing monitoring of mental health and addiction issues in clients
- Develop and implement individualized treatment plans based on client needs
- Provide support to adults clients of all ages in their daily living routines in various community settings as needed
- Provide counseling and support to clients, and assist them in developing coping and life skills
- Work collaboratively with other healthcare providers to ensure continuity of care for clients
- Provide crisis intervention as needed, and develop safety plans with clients
- Assist clients in accessing community resources, such as healthcare, employment, and education
- Maintain accurate and up-to-date clinical documentation
- Participate in case conferences and team meetings to discuss client progress and treatment plans
- Provide education and support to staff, clients, and families on mental health and addiction issues
- Stay current with industry trends, developments, and evidence-based practices
- Facilitate staff orientation on home management (housekeeping, meal preparation) and client support duties (personal care, behavioral and emotional support). We prefer someone with previous experience and a positive attitude.
- Facilitate wellness and recreational activities to support the wellbeing of a diverse clientele in various community settings
- Execute tasks aligned with company’s policies and procedures
- May be assigned other duties as required

Qualifications:

- Certificates or Degrees in social work, occupational therapy, psychology, counselling, psychiatric nursing, mental health and addictions or a related field
- Registration or eligibility for registration with a professional college or association
- At least 2 years of experience in mental health and addictions
- Experience working with individuals at risk/ experiencing homelessness, mental health challenges, addictions, chronic illnesses, trauma, or in vulnerable circumstances (required)
- Strong knowledge of evidence-based interventions and treatment approaches
- Excellent clinical skills and experience in assessment, treatment planning, and intervention
- Experience working in a supportive housing environment
- Strong communication and interpersonal skills
- Ability to work collaboratively with interdisciplinary teams
- Experience with electronic medical records and documentation
- Willingness to participate in quality improvement initiatives and stay current with industry developments
- Demonstrate a good working knowledge of the technical details important to the functioning of supportive housing programs
- Updated First Aid/ CPR (required)
- Non-violent crisis intervention training desirable
- Knowledge of trauma-informed practice
- Knowledge of harm reduction
- Knowledge of Mental Health First Aid
- Knowledge of WHIMIS
- Knowledge of Food Safety Basics
- Knowledge of Indigenous cultural diversity and Canadian multicultural context, LGBTQ2S competent
- Knowledge of community resources
- Clear police check for the vulnerable sector (required)
- Recent negative TB test and updated immunizations preferred
- Valid driver’s license and reliable vehicle (required)

**Job Types**: Full-time, Part-time
Part-time hours: 30 per week

**Salary**: $18.00-$22.00 per hour
