Acclaim Health announces the availability of an Established Part Time Care Manager, Memory Visiting position with the Community Support Services department working in the Oakville Office. The incumbent will work Monday to Thursday from 8:30am - 5:00pm.

**DUTIES & RESPONSIBLITIES**:

- Participates in the development and implementation of the strategic and operational plan for the Memory Visiting Program, Community Support Services.
- Conducts initial in-home client assessments to determine eligibility and to assess specific clients’ needs.
- Works in a manner that ensures client safety.
- Responsible for the supervision, training, mentoring and development of volunteers and students within the specific program.
- Matches and assigns volunteers to clients according to identified needs
- Monitors client/volunteer relationship at regular intervals and conducts follow up visits to clients and volunteers, making changes as needed and when appropriate
- Ensures the timely provision of information and supportive care to clients and family caregivers; refers clients to other resources as appropriate.
- Collaborates with other members of the clients’ care team and participates in client care conferences as needed
- Collaborates with Director in evaluating client services in accordance with client-focused expectations and continuous improvement.
- Gathers statistics for monthly and quarterly statistical reports as they relate to specific program.
- Assists with planning and coordinating educational sessions for volunteers.
- Participates in the organization of volunteer recognition events and special events for clients.

**QUALIFICATIONS & EDUCATION**:

- Post secondary education in Gerontology, Social Services or related discipline.
- Volunteer management certificate as asset.
- Certification in CPR-Basic Rescuer level.
- Valid Ontario Drivers License and insurance coverage in accordance with the company policy.

**EXPERIENCE & SKILLS**:

- Minimum 2 to 3 years demonstrated experience working with adults with dementia and other cognitive impairments
- Ability to work effectively with volunteers.
- Good verbal and written communication and interpersonal skills.
- Knowledge and understanding of volunteerism, community resources and Halton health care system.
- Effective decision making and problem solving skills.
- Good organizational skills.
- Strong assessment and interviewing skills.
- Well-developed knowledge and skills in quality/risk management.
- Proficient in Microsoft Office 365
