South Riverdale Community Health Centre is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. The Health Promoter - Choose Health coordinates and implements the health education/ promotion/social prescribing programs for the community and facilitates the integration of a health promotion focus in CHC activities as a member of a multi-disciplinary team. This position works collaboratively with community groups and agencies using a community development model. This position is further expected to be available at hours that are suitable for community members and volunteers. This may mean early morning and evening hours as well as weekend time.
This position is expected to work onsite during the COVID-19 pandemic as required.
**Responsibilities**:
- Lead the development, planning and implementation of Self-Management Programs
- Act as a facilitator as required and function as Master trainer for Chronic Disease Self-Management Program and Chronic Pain Self-Management Program and other programs as required
- Anchor partner liaison, supervision of peers, partner recruitment and outreach, and peer facilitator mentor/support
- Oversee and recruit program facilitators
- Explore service partnerships in collaboration with management team
- Network with internal and community partners to ensure referral to available self-management patient education programs and healthcare provider trainings
- Develop and implement outreach strategies to connect with marginalized community members living with long term health conditions in order to refer self-management programs
- Implement service provision in an appropriate and culturally competent manner to a culturally and socio-economically diverse range of communities
- Work to develop a communications/marketing strategy to promote awareness of the SMP and related activities among individuals with or at risk of chronic disease including diabetes and their health care providers
- Lead program team in production of program material, brochures, and flyers
- Routinely maintain and update selfmanagementtc.ca to promote upcoming chronic disease programs/events
- Identify, recruit, and organize all self-management training
- Supervise, support, and mentor Health Care Provider facilitators and Peers with lived experience
- Develop and implement strategies to maintain an active and engaged trainer/volunteer peer leader/mentor community
- Facilitate ongoing peer facilitator training and coaching sessions
- Recruit participants and support partner organizations with participant recruitment
- Represent the program with strong knowledge of overall program especially when developing new partnerships in conjunction with management team
- Coordinate program evaluation activities including audits completed by master trainer with support of health promoter or health promoter in capacity as master trainer
- Collect, summarize and report data aligned with the initiative’s deliverables, including evaluation data
- Provide support to participants and facilitators including form completion, systems navigation, advocacy and social prescribing and referrals to address needs that are not therapeutic or medical
- Lead Choose Health team in collaborating with other teams across the organization to identify and provide self-management health education to target groups
- Establish community development initiatives in alignment with SRCHC’s vision
- Participate as a team member in all team functions program planning, team meetings, facilitator meetings, and partner meetings
- Support delivery of virtual and in person workshops as needed
- Analyse data and prepare program reports as per program directives and funder/organizational requirements
- Write funding proposals to secure additional resources to support the Self-Management Programs
- Oversee processing of program invoices for manager signature
- Oversee program participant referral and booking system and establish protocols
- Oversee and maintain paper and electronic file management systems following established protocols
