For 50 years Nellie’s has been providing shelter and support for marginalized women and their children in the Greater Toronto Area. We operate programs and services, for women and their children who had or are experiencing violence, poverty, and homelessness. Nellie’s works to lessen homeless for women and their children whether the cause is family violence, mental health, or any other reason that results in dealing with the impact from trauma.

Nellie’s operates Community Support and Outreach programs that provides after care support and follow-up services to women and their children after they leave the shelter, as well as transitional housing support for women living in the community who are homeless or at-risk of becoming homeless.

The contract Transitional Housing Support Worker is a part of a Transitional Housing Support Program (THSP) Expansion pilot project to provide outreach support to women and gender diverse people who are currently experiencing or who have experienced gender-based violence who may also be experiencing mental health and substance use challenges. This project prioritizes supporting individuals and families in the Toronto shelter system who need gender-based violence support. The pilot project is a collaborative partnership between Nellie’s and Ernestine’s.

**JOB SUMMARY/DESCRIPTION**
- **Support Women to Find and Maintain Housing**:

- Develop an updated list of appropriate housing resources.
- Develop and maintain network of relevant contacts in the housing sector.
- Provides appropriate, timely information to women regarding housing alternatives
- Supports women to select housing options.

**Advocacy**
- Assist clients with family, criminal and immigration legal matters
- Assist clients with forms and documentation that needs to be completed
- Create and maintain a list of lawyer referrals who accept legal aid certificates
- Keep current with changing laws and trends as they relate to family, criminal and immigration issues
- Advocate for clients with law enforcement, lawyers, and other agencies.
- Provide escorts to legal appointments and court appearances.
- Arrange for cultural interpreters for all appointments and court date
- Liaison and navigate the victim services programs
- Assist in claims to the criminal injuries compensation brd.

**Wok with Women to Prepare Safety Plans**
- Develop safety plan tools.
- Develop safety plans with all clients.

**Maintenance of Participants Management System**
- Develops and maintains participant’s database, files and records.
- Tracks and analyzes data.
- Reports on and analyzes trends in the program.
- Prepares program reports as necessary
- Works with management team to identify and prepare funding proposals.
- Develops and maintains resource library.

**Assist Women to Establish a Network of Support in their Community**
- Develops and maintains update information or relevant resources in the communities were housing is available.
- Develops and maintains community partnerships.
- Advocates on behalf of women.
- Coordinates referrals for services and supports to outside agencies.
- Maintains regular contact either in person or by phone.
- Outreach and network to organizations in the above mentioned list.
- Escorts women into her new community.
- Support women to access appropriated resources in her new community.
- Provides transitional support upon and after discharge from shelter.
- Community Education
- Represents the agency within the community and with partners, stakeholders and other constituents.
- Builds relationships and linkages with housing support communities.
- Liaises with agencies involved with client.
- Teambuilding
- Contributes to organization team building process.
- Participates in case conference meetings to review women’s goals, outstanding needs and develop case plans, identify expected outcomes.
- Consults and coordinates client process with other appropriate staff.
- Works independently and also as part of a larger team to identify client needs and assist the client in accessing appropriate programs and services.

**Qualifications**
- 2 or more years demonstrated success in planning, coordinating and evaluating programs
- Experience in and commitment to working within an anti-oppression framework
- Experience in the social service sector ideally in an agency delivering counseling, crisis intervention or advocacy services
- Able to work occasional nights and weekends
- Consistently demonstrates reliable professionalism in working in, out and across the organization.
- Proven commitment to AR/AO
- Experience in Community
