Under the direction of the Director of Housing, the Life Skills Coach, using a harm reduction approach will provide a broad range of holistic services to support, promote and empower constituents who are homeless, at risk of homelessness, marginalized and living in social isolation with supports to assist them along their journey to address barriers to success. The Life Skills Coach provides a holistic approach that motivates and facilitates adjustment to change and learning opportunities through groups and individuals consultation. They prepare constituents to deal effectively with personal issues by guiding them activities such as self discovery and behaviour identification exercises. The Life Skills Coach facilitates an environment that builds on self esteem, empowerment and self determination that encourages independence and self confidence. It is expected that the Life Skills Coach have the ability and skills to work with constituents who may be hostile, oppositional and volatile, while at all times maintaining composure and behave in a professional and dignified manner.
The Life Skills Coach shall ensure that all activities are undertaken in a manner that is consistent with, and complementary to, the Vision Statement, The Principles and Values and the policies and procedures of the Aboriginal Health & Wellness Centre.
**DUTIES & RESPONSIBILITIES**:
The Life Skills Coach is responsible for the implementation, instruction, and coordination of Life Skills to assist constituents to gain successful integration within the community.
In collaboration with the constituent identify goals for development. Assess the constituent’s readiness to make reasonable change to their current circumstance and evaluate their willingness, attitude and commitment to make the necessary changes for success.
Ability and skills to work with individuals who may be hostile, oppositional and volatile, while at all times maintaining composure and behave in a professional and dignified manner.
Network with Community Resources to provide continuing learning opportunities to supplement and improve constituent life skills success
Research and develop materials necessary to supplement constituent Life Skills.
Collaborate with constituent to develop the appropriate skill necessary to assist them to identify strategies, self realization tools and techniques towards their journey of success.
Collaborate with constituent to foster the development of new and or improved coping, problem solving, and communication skills.
In consultation with the team make recommendations in regards to referral needs of the constituent.
Develop, implement and facilitate individualized training and or group workshops to address the needs of the constituent.
Formulates and delivers support, treatment and healing strategies, based upon the individual beliefs and desires of the constituent, and including the provision of ongoing input and monitoring of progress, which utilizes an appropriate combination of traditional methods and values partnered with access to contemporary physical, emotional and psychological services. Such services and other needed services may be provided to constituents in any environment including: the streets, shelters, correctional facilities, hospitals, apartments, office, etc.
Maintain written and computerized records, compile reports and complete other program documentation (e.g., S.O.A.P. notes, progress reports, incident reports, on-call logs, letters, assessments, etc) as required.
Coordinate and monitor referrals to community services and advocate constituent participation.
Facilitates, and provides support to, individual and group activities such as personal counseling sessions, Sharing Circles, Sweats and Retreats which are designed to provide a base of personal knowledge and reference to constituents through individual and shared interactions and discussions
Maintains a thorough and accurate record of all constituent contacts, personal visits and appointments, agency referrals and document progress towards achieving the goal(s) and objective(s) established collaboratively with each constituent
Is an advocate with other agencies, organizations and individuals for the purpose of developing self-sufficiency and independence on behalf of constituents.
Adheres to the definition of, and reporting procedures as legislated by the Province of Manitoba i.e. the Child and Family Services Act etc.
Organize and participate in social activities with constituents in the community.
Perform other job related duties as assigned.
**QUALIFICATIONS**:
Master’s Level or a Bachelor’s Degree in Social Science and two years of facilitation skills in the delivery of individualized and or group workshops, or a combination of education
