Reports to Program Manager and Community Health Coordinator
**PAQ Mission**
PAQ is an Indigenous organization that supports First Nations, Inuit and Métis people facing housing
insecurity in Tio'tià:ke / Montreal. Using a culturally appropriate approach based on empowerment
and harm reduction, we provide housing options, housing, and services that promote well-being and
healing, and strengthen community. We work in partnership with others to promote the health and
housing rights of urban Indigenous peoples.
**Job Purpose**
As a part of the PAQ team, you will be joining a group of individuals who are committed to helping
change the lives of Indigenous men and women in Montreal. Working closely with community
members to meet their long-term health goals, you will have the opportunity to see the life changing
benefits of your work on a daily basis. Working with dedication to PAQ’s mission, you will have access
to the support of team members as well as training in the areas of intervention, suicide prevention,
trauma informed approaches, First Aid and Indigenous cultural knowledge.
**Duties and Responsibilities**
The person in this role works in close collaboration with the Community Health Coordinator and in
- hospital liaison officer at all the hospitals. To support First Nations, Métis and Inuit women and men
experiencing homelessness or at risk to help them access holistic and culturally adapted physical and
psychological health services. The role will require assisting the Community Health Coordinator, the
Community Health Support Worker and in-hospital liaison officer.
**Main duties**: To visit all hospitals and emergency units, to support the urban indigenous population,
to make sure all follow-up appointments, intakes, consent forms are given to the Community Health
Coordinator. The role will present a unique combination of outreach and accompaniment for
participants of the program. Working with individuals with chronic addictions, and mental health
challenges, and serious illnesses can be part of the workload that requires strong crisis management
skills. This person uses a culturally appropriate approach that encompasses Indigenous traditions,
culture and worldview.
Overall, the role of PAQ Community Navigator is:
To support and accompany Indigenous patients in their care pathway from a perspective of cultural
safety by:
- Offer cultural and emotional support
- Act as cultural interpreter to allow Indigenous patients, health and social services.
- Ensure that they understand the information about their health condition, the care and
services offered. Inform users of resources, programs and services that are available at PAQ
and offer support for specific service requests.
- Work with PAQ and other Indigenous organizations in Montreal and the liaison officer in
- hospital to identify required improvements to provide cultural safety for people they serve.
- Advocate for the indigenous patients with respect to all
- Build knowledge and networks of other partner services, programs, and resources to
support referrals, and follow-up.
- Establish a bond of trust with participants through active listening and positive interactions
**Specific administrative duties**:
- In collaboration with the participant, the Community Health Coordinator and the hospital Patient
Liaison design an action plan tailored to their individuals and realities and that meet their health
needs.
- Maintain a record on the daily tracker and submit information to Coordinator at the end of
each week, good communication with the Community Health Coordinator.
- Other duties as assigned.

**Values, Experience, Skills, Qualifications**
- Minimum 2 years of work experience in a similar intervention / social work role or other
related fields, preferably in the Indigenous community sector or with people experiencing
homelessness or the health service sector.
- Knowledge and understanding of the impact of intergenerational trauma and ongoing realities
in Indigenous communities, urban barriers, and Indigenous social issues.
- Basic knowledge and understanding of healthcare (hospital, private, or community setting) or
strong willingness to learn.
- Experience as a facilitator and/or trainer.
- Bilingualism is preferred; knowledge of at least one Indigenous language is an important asset.
- Ability to effectively use Microsoft Suite (Word, Excel)
- Experience working in the community sector; experience working with Indigenous
communities or within the homelessness sector is a distinct advantage.
