**Position Summary**

**Care Transitions Coordinator, Patient Flow**
Your goal in this position is to coordinate and provide support through facilitating and coordinating all aspects of patients flow and transitions from admission, to assignment, to discharge in collaboration with the appropriate allied health professional(s) to ensure the appropriate placement with the required care in the community. You will greatly contribute to NYGH’s mandate to put patients first in everything we do by ensuring patients receive safe, timely, appropriate and integrated care throughout their journey. You will be collaborating with our community partners, LHIN, Ministry and other care providers which would reflect positively on our organizational performance, compliance and capacity-building work. This position supports the hospital’s academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, volunteers and families.

**On a practical level, you will**
- Provide coordination and support to patient admissions, discharges, transfers and transitions utilizing NYGH’s flow and access tools, standards, protocols by working collaboratively with physicians, Bed Control Specialist, Social Workers, LHIN, community providers and others as required.
- Facilitate the appropriate placement of patients through working and communicating directly with all hospital clinical and non-clinical teams, including leading daily patient placement meetings, to improve the patient experience related to Time to Inpatient Bed, access to care and overall patient flow.
- Work collaboratively, independently and effectively to assess patient transition needs and challenges, and identify and mitigate barriers to appropriate patient discharge and care transitions, with physicians, Social Workers, Unit Coordinators, LHINs, Home and Community Care, and other members of the care teams both internally and externally.
- Support the discharge planning and workflow optimization for Resource Matching and Referrals (RM&R), iPlan Platform, Alternate Level of Care (ALC) patients as well as various integrated and bundled care models, working in collaboration with various clinical professionals.
- Capture and track data related to patient flow and transitions operations and develop KPIs to evaluate performance.
- Verify, correct and submit data and compliance reports pertaining to NYGH’s patient flow activities, occupancy, ALC, etc. as required.
- Actively lead and support continuous quality improvement projects and initiatives as they relate to Patient Flow and Care Transition.

**Qualifications**
- Undergraduate degree in a health care discipline required.
- Current member and in good standing with a professional college required.
- Minimum 3 years’ recent clinical experience in an acute care hospital setting required.
- Project Management (PMP Certification) (preferred).
- Lean Six Sigma certification (preferred).
- Ability to work independently and effectively to assess patients’ placement needs.
- Proven leadership skills.
- Data quality management and performance evaluation.
- Strong facilitation and conflict resolution skills to help resolve challenging situations.
- Ability to assess risk, and effectively utilize resources to support patient acuity.
- Bilingualism in French/English an asset.
