**Position Overview**:
The Primary Health Care service area provides regional coordination and support within the WRHA and operates centralized PHC services. Areas of emphasis include quality improvement and risk management, information management, strategic planning, system wide service coordination and integration, operational standards pertaining to community and primary health care. The Primary Health Care service area includes WRHA Primary Care Clinics (including all sites where teaching occurs), community health agencies funded by the WRHA, Community Development, Antenatal Home Care and low risk obstetrics (Midwifery Services). The Program also leads primary care renewal initiatives in collaboration with many partners across the health system. Key Primary Care renewal activities include My Health Teams, Interprofessional Team Demonstration Initiative (ITDI) and Family Doctor Finder.

**Experience**:

- The incumbent requires a minimum of four years related experience working in the area of service delivery, business operations and have demonstrated experience in service coordination and integration. The incumbent will demonstrate a sound understanding of population health, health promotion, community development and primary health care.
- In addition, the incumbent will demonstrate experience in the following areas:

- Service planning, development, implementation and evaluation with a particular emphasis on service delivery Knowledge and experience in the area of human resource management, budget and financial and statistical reporting systems
- Knowledge and understanding community development principles and experience.
- Experience in promoting health from a health equity lens.
- Experience working within a community-based organization with a diverse range of community groups and with vulnerable populations.
- Experience in promoting health through community mobilization and community building.
- Experience managing relationships with WRHA community funded agencies and managing the overall agency accountability processes of these funding relationship through Service Purchase Agreements.
- Experience working in private family medicine practice and working with private practice physicians is an asset.

**Education (Degree/Diploma/Certificate)**:

- Bachelor’s Degree in a relevant discipline (e.g. health related discipline); Master’s Degree in a related area required.

**Certification/Licensure/Registration**:

- Responsible for maintaining and providing proof of active registration with the appropriate registering body.

**Qualifications and Skills**:

- Excellent organizational, interpersonal, problem solving and decision-making skills.
- Demonstrated leadership skills in an interdisciplinary team environment.
- Ability to work independently within an interdisciplinary, intersectoral and intrasectoral team.
- Excellent written and oral communication skills including experience with proposal writing and giving presentations.
- Ability to evaluate evidence and literature.
- Ability to work with multi-sectoral partnerships within and beyond the health services sector.
- Ability to liaise with other jurisdictions and organizations on a provincial and national level.
- Knowledge of existing community resources providing services in Winnipeg.
- Ability to communicate and work effectively across all WRHA service areas and sectors.
- Strong understanding of the key strategies and initiatives for Primary Care Renewal in Manitoba and a sound understanding its key pillars.
- Understanding of the strategic vision of supporting the development of a primary care system within the Winnipeg Health Region and the key building blocks to achieve this objective.
