- OVERVIEW_

The Street Outreach Case Manager is responsible for the day-to-day operations and activities of the Street Outreach Program. This includes creating and building working relationships with individuals sleeping rough, connecting them to appropriate services, including but not limited too; access to emergency shelter, available housing options, mental health and addiction support and basic needs assistance. In addition, the Street Outreach Case manager will liaise with community partners to create appropriate referral mechanisms. The Street Outreach Case Manager works closely with the Program Manager in the on-going development of the program to meet client and community needs.
- DESCRIPTION_

**The Street Outreach Case Manager will**:

- Completion of program statistics for regular review with the Program Manager. Assist with the development of an internal system for profiling clients and outcomes.
- Identifies procedures and processes that need improvement and recommends corrective action by collaborating with the emergency workers
- Develops relationships with other services for individuals in the community to understand community gaps and work toward common solutions; to develop referral mechanisms and other joint programming as appropriate
- Establishes rapport with individuals living on the street, assist in educating around community services and connects clients to appropriate services
- Reports any major or urgent concerns related to safety and security to the Program Manager.
- Monitor and review database entries for clients, ensuring records are up to date.
- Identify new locations frequented by homeless individuals and ensure outreach teams are providing service in these areas

**Required skills and Knowledge**
- knowledge of homelessness and affordable housing issues within the community and the strategies to end chronic homelessness which are identified the City of Kingston’s _10-year Housing and Homelessness Plan_ (Dec 2013);
- knowledge of the operating standards for the program and the program’s expected client outcomes;
- knowledge of the resources in the community for individuals, youth and families who are homeless, at risk of homelessness, living in poverty, or living with mental health or addiction issues and skilled at brokering services for clients appropriate to the responsibilities of the job;
- ability to integrate harm reduction approaches as appropriate in interactions with the client;
- ability to use assertive engagement techniques as appropriate in interactions with the client;
- knowledge of and adherence to privacy legislation and policies within the organization related to confidentiality of client information;
- ability to identify and assess risk and implement strategies which minimize risk to self, the client, and others;
- strong problem solving, conflict resolution and mediation skills within a solution-focused framework;
- excellent communication skills, both written and verbal
- the ability to negotiate and mediate difficult situations and produce positive results
- ability to function as a participating member of the team and understand the work of the team as part of a broader system to end chronic homelessness
- background in systems work, data analysis and supporting the work of others to achieve system goals;
- supports a learning culture that stays up to date with the main currents of thought and practice in the delivery of housing first programs
- develops the staffing schedule in consultation with the team to ensure maximum efficiency while meeting program objectives.
- promotes the development of the team and deals with team issues promptly.

**Qualifications**
- University Degree or Community College Diploma in a related discipline
- Minimum five-years work experience in human services, of which at least three years shall be in services targeted to the homeless population
- Demonstrated ability in both oral and written communications
- Strong negotiation and mediation skills
- Demonstrated knowledge of issues facing street involved individuals
- Demonstrated organizational abilities
- Valid Driver’s License and access to a car

**Shift Schedule**

Shifts will consist of days, evenings, and weekends.

**Job Types**: Full-time, Permanent

Pay: $50,372.00-$61,842.00 per year

Schedule:
