**POSITION**: Passport Worker - Social Services
**LOCATION**: Barrie with responsibility throughout Simcoe County

**PURPOSE OF POSITION**
To facilitate the Passport Program using Wraparound Principles for individuals and families in Simcoe County, working in conjunction with Passport One and Developmental Services Ontario.

**STATEMENT OF MAJOR REPSONSIBILITIES**
- To support the allocation of Passport Funds to recipients through the provision of information and required documentation.
- To Communicate with individuals, Person Managing Funds and community support teams about their funding allocation, eligible expenses, documentation requirements, invoice submission practices, etc.
- To provide information upon request to recipients/Person Managing Funds and staff regarding community participation and respite resources available in their communities, as well as eligibility of expenses.
- To support the funding entity, Passport One, in processing invoices by monitoring and resolving invoices issues, exception requests, change in personal information such as address and Person Managing Funds, in addition to data entry maintaining the provincial database utilized by Passport One.
- To maintain Passport Service Data in the provincial Developmental Services Consolidated Information System (DSCIS) database. Data entry, monitoring and upgrading of information and reports required to meet provincial data requirements.
- To disseminate information to recipients/Person Managing Funds, service agencies, agency staff and other stakeholders regarding the Passport Program.
- To assist Passport recipients to develop a plan for their services, and provide information and assistance to begin the implementation of their plans as required. Assist recipients with Person Directed Plans as required. Prepare individual agreements in consultation with the fund administrator and facilitate and obtain signatures required.
- Maintain service data to ensure accurate reporting and statistics through agency maintained records as well as provincial databases.
- To attend and participate in supervision, team meetings, performance appraisal.
- To participate in professional development in consultation with Supervisor/Manager, and demonstrate initiative in learning and developing new techniques of service delivery.
- To practice effective communication with co-workers, consulting and sharing resource information and providing case coverage as required.
- To participate in committee work, internally or externally, as assigned.
- To adhere to all agency standards and guidelines and procedures including health and safety.
- To perform such other duties as may be assigned by the Supervisor/Manager that are consistent with the incumbent’s position.

**QUALIFICATIONS**

**Education and Experience**
- Generally acquired through combination of post-secondary education at a Community College level in one of the social or behavioural disciplines plus two years related work experience.

**Knowledge and/or Skill Level**
- Ability to develop trusting, co-operative relationships with clients, families, service providers, and other stakeholders and to interact with individuals with special/exceptional needs.
- A commitment to Wraparound approach.
- Excellent computer skills, working knowledge of Excel, familiarity with internet-based platforms.
- Excellent communication and negotiating skills and judgement.
- Knowledge of community resources, family systems and relevant legislation for individuals with special needs.
- Sensitivity, understanding and ability to work with a variety of value systems, beliefs and cultures.
- A working knowledge of French Language is beneficial

**Valid driver’s license and access to a reliable vehicle.**

This position is within the bargaining unit.

**Hourly Rate**:$26.43- $29.73 per hour, plus a generous Employee Benefit Package, including a Pension & Health and Dental Plan.
