**Hours: 9 a.m. to 5 p.m. Monday to Friday with some evening**
**hours depending on group schedule**

The Canadian Mental Health Association, Halton Region Branch (CMHA-HRB) is seeking a Full-Time Peer Transition Navigator for the PHAST (Prioritizing Health through Acute Stabilization and Transition) program for a 1 year contract, which is a collaborative partnership with ADAPT, ROCK, JBH and CMHA. PHAST is a short-term, transitional program that supports acute stabilization of acute crises through group-based supports, and system navigation. Peer Support is provided by a trained individual who has their own lived experience of having a mental health concern and/or substance use/addiction concern.

**About the PHAST Program**

The PHAST program is an outpatient, hospital-based program that strives to support people who are in crisis to:

- Enhance coping strategies, life skills and social skills
- Have a strong network of reliable and supportive relationships
- Receive a client centered approach to service
- Prevent inpatient admission, reduce recidivism to the Emergency Department, and reduce length of stay when inpatient care is necessary

**What is PHAST? Outpatient Crisis Program**

A program that will consist of:

- Rapid Mental Health & Addictions Assessment & Treatment targeting acute stabilization
- Emphasis on group support and solution focused individual care
- Warm transition Support/Navigation from acute supports to next stage services
- Seamless transfer from episodic crisis management to comprehensive community-based care

**Responsibilities**

**Peer Transition Navigator (60%)**:

- Act as a role model in self-care, self-awareness and wellness tools via values-based practice
- Provide 1:1 transition support and warm transfer to next stage services
- Share common experiences
- Support peers to address areas of concerns and explore solutions
- Focus on strategies to empower peers to integrate into the community
- Support peers to establish their own goals
- Promote and facilitate education and awareness of peer support and person-directed care to peers, co-workers and in the community as require

**Transition Navigator/Case Management (40%)**
- Provide intensive short-term support to individuals (1:1) in the community with crisis intervention, service coordination, recovery planning and facilitate stabilization help to clients with significant mental health and/or addiction issues
- Make referrals on behalf of clients and facilitate connection to relevant community services
- Facilitate and deliver rotating schedule of groups in collaboration with team members
- Support individuals within the mental health system by empowering them, identifying goals and strengths and successfully implementing formal and informal supports
- Responsible for maintenance of client records
- Work collaboratively within PHAST program (i.e. work alongside Psychiatrist and complete required documentation) and community partners in a multidisciplinary approach
- Other duties as assigned

**Required Skills & Experience**:

- Personal experience with mental health concerns and/or substance use/addiction concerns and active in your recovery journey
- Working experience within a crisis environment is an asset
- Alignment with the Core Values of Peer Support
- Comfort sharing your own experiences with others to inspire hope
- Community outreach and knowledge base of community resources and services
- Able to work independently and co-operatively in a busy multidisciplinary environment
- Strong oral, facilitation, and written communication skills
- Initiative, creativity, flexibility and an openness to change

**Qualifications**:

- Diploma or Bachelor’s degree in social work, nursing, occupational therapy or a relevant field preferred
- Training in Peer Support Work (Ex. Core Skills Training, Core Essentials - OPDI)
- Non-violent crisis intervention, first aid, CPR and ASIST certifications are an asset
- Comfortable in the delivery of evidence-based informed approaches
- WRAP Training Certification is an asset
- Proficiency in MS Word and Excel
- The position occasionally entails the transportation of peers. A valid Ontario Class “G” driver’s license and a vehicle with 2-million-dollar insurance coverage is required.
