The West Toronto Ontario Health Team (WTOHT) is looking for an Operational Lead, Primary Care. Our vision is to improve the experiences of, and outcomes for, people living and seeking care in West Toronto for all stages of life by becoming one connected system of health and social care.

Under the direction of the WTOHT Director and working alongside the primary care clinical leadership and Co-Chairs of the Integrated Primary Care Working Group, the Operational Lead, Primary Care will be the dedicated resource for the Integrated Primary Care Working Group, West Toronto Primary Care Network (WTPCN) and West Toronto Primary Care Council. This position will be responsible for advancing the implementation of Ontario Health’s primary care guidance, overseeing the growth of the West Toronto Primary Care Network and co-designing and supporting integrated primary care initiatives (i.e. Health & Wellness Centres in Toronto Community Housing locations, clinics to support ED diversion).

**Responsibilities**

The Operational Lead, Primary Care Network will be responsible for:

- The development and implementation of the Integrated Primary Care Working Group’s annual work plan in collaboration with Working Group;
- The development and implementation of the West Toronto Primary Care Network’s annual work plan in collaboration with the WT-PCN leadership;
- Supporting the advancement of integrated primary care initiatives in West Toronto;
- Facilitating engagement with primary care providers, service providing organizations, patients and caregivers, and other stakeholders across West Toronto to support advancement of WTOHT priorities and objectives;
- Organizing virtual and in-person events to engage primary care providers;
- Preparing content for the WTPCN’s monthly newsletter;
- Overseeing outreach to primary care providers in West Toronto in preparation for future health human resource capacity planning;
- Supporting planning and decision-making through population health data and primary care database; and
- Other responsibilities that arise as needed.

**Qualifications & Key Skills**
- Minimum undergraduate degree with a preference for specializations related to health system planning and/or primary care. MPH, MPA, MHA or graduate level education are all considered assets for applicants;
- Minimum 3-5 years of direct experience in the primary care sector and/or working directly with family physicians and primary care providers;
- Creative problem-solver eager to work collaboratively with others;
- Ability to facilitate discussions, workshops and other planning sessions with primary care providers, service providing organizations and other stakeholders;
- Ability to break down complex system problems into a sequence of manageable work packages;
- Experience successfully leading and supporting change projects;
- Experience working in a collaborative leadership environment and managing diverse project stakeholder relationships and expectations;
- Strong digital and computer literacy;
- Experience working independently to achieve deliverables in collaboration with internal and external stakeholders; and
- Demonstrated cultural competency while working with people of diverse cultural, age and economic backgrounds as well as marginalized individuals.

**Job Type**: Full-time permanent role

**Hours of Work**: 37.5 workweek with flexibility to accommodate early morning and/or evening meetings to adapt to physicians’ clinical schedules. The incumbent may be required to work beyond 5:00 pm and is expected to attend periodic evening meetings with key stakeholders.

**Salary**:$90,000 - $110,000 per annum plus paid vacation days, 10 paid professional development days and excellent benefits.
