We are currently looking to hire a:
**MANAGER, PRIMARY CARE - 14 month contract - Full Time (35 hrs/wk)**

**Reporting To**:Vice President Community Health and Professional Practice

**Purpose**:
The Manager of Primary Care will provide day-to-day operational leadership for the
Community Health Centre primary care teams and delivery of high quality programs and services that responds to the needs of the community across multiple sites in Scarborough.

**Key areas of responsibility**:

- _Leadership and Interprofessional Care:_
- Understands, interprets, and implements SCHC’s mission, vision, and values
- Promote person and family centred care in practice with a strong emphasis on an interprofessional team approach, and supporting each discipline to work to their full scope of practice within the primary care team.
- Act as a leader in interprofessional team-based care, providing evidence-based, comprehensive health care emphasizing healthy living, and illness prevention specific to client population needs
- In collaboration with the VP provide input into the clinical program outcome evaluation activities
- Assist the VP in identifying new clinical service development, implementation, and evaluation opportunities.
- Works in collaboration with the Manager Community Health Team
- In collaboration with the VP, manage the team budget.
- Ensure continuous quality improvement by consistently analyzing CHC data and making recommendation on program improvement
- Design and implement program improvement ideas by continuously monitoring key health care trends in the community
- **Team Development and Orientation**:_
- Develop and implement individualized plans for professional growth and development, including participation in professional organizations and activities, workshops, seminars, and staff development programs
- Monitors the interprofessional team practices ensuring compliance with relevant legislation and applicable standards of practice
- Provide supervision, mentorship, orientation, observation, and teaching opportunities to the interprofessional team members
- Participate in interprofessional meetings, task forces and projects.

- Participate in the planning and development of Primary Care policies, medical directive, procedures, and care protocols and the implementation of interprofessional standards of practice as it relates to the different disciplines in the primary care teams.
- **General Responsibilities and Work Plan**_
- Supports the VP in implementing the work plan for primary care
- Assists the VP in meeting the standards for accreditation
- Develop and maintain professional competence
- Responsible for staff scheduling, and primary care provider on call schedule
- Redesigns work flow processes to ensure efficiency and access for clients
- Perform other duties as assigned that are reasonable within the scope of the job.

**Scope and Accountability**
The manager role supports the development, coordination and provision of primary care services across the community health centres are in accordance with legislative and regulatory requirements, professional standards, and the policies of SCHC.

**Educational and/or Professional Qualifications**:

- Certificate of registration in good standing with the College of Nurses of Ontario
- Baccalaureate of Science in Nursing, Health Administration, or relevant field
- Member of the Registered Nurses Association of Ontario

**Level of Experience**:

- 1-3 years of experience managing in a interprofessional and culturally diverse health care environment
- Experience supervising staff, physicians, students and volunteers
- Experiencing in determining appropriate staffing patterns, skill mix, recruitment and human resource planning

**Skills and Attributes**
- Demonstrated clinical leadership skills
- Developed critical/strategic thinking skills
- Excellent interpersonal skills with a strong orientation to staff and team growth and development
- Ability to create a collaborative multidisciplinary working environment which fosters high morale and effective staff relationships and participation
- Demonstrated knowledge and commitment to the principles and practice of health promotion, harm reduction and the social determinants of health
- In collaboration with the VP, develops the program strategic human resources plan including short and long term needs;
- Directs the investigation and resolution of patient complaints;
- Ability to work between multiple sites due to CHC locations
