Coordinated Access for Addiction Services provides call center screening and triage services for individuals and families requiring counseling, information and referrals to Toronto Central Local Health Integration Network (TCLHIN) funded addiction services across the City of Toronto. The Coordinated Access Caseworker also manages the Central Access phone service, providing screening and referral to Withdrawal Management Services in the city of Toronto.

The Coordinated Access Caseworker (CACW) provides crisis intervention, counseling, case management, referral, and treatment planning, from a trauma informed care perspective to individuals and families. The role is performed on the telephone. The CACW works with complex and sensitive cases requiring a sophisticated understanding of: most aspects of Substance Use and Concurrent Disorders, different cultures and various populations. The CACW collaborates with: colleagues in integrating knowledge with practice, other agencies and service providers to address challenges like barriers to service and gaps in the system. Participates in developing, implementing or evaluating large or complex programs, or leads such efforts for smaller or less complex programs/initiatives.

**DUTIES & RESPONSIBILITIES**:
Establishes and maintains therapeutic rapport with Clients, on the telephone or in person, to facilitate commitment to adhering to treatment plans and establishes and maintains treatment plans as part of a multi-disciplinary team, as appropriate.
Provides short term face to face and transitional support for individuals as required.
Employs evidence-informed health screening and assessment tools to identify health problems that might influence the Client’s treatment plan.
Records data related to calls and analyzes data and enters data into reports.
Presents to community agencies on the role and function of Coordinated Access.
Assess and monitors Clients at risk of suicide and/or other Substance Use related harm and initiates appropriate interventions or referral as required.
Assess the relative severity of the Client’s Substance Use and Mental Illness or Mental Health Issues, if present, and adjusts the treatment plan to reflect the relative priority of each.
Creates treatment plans that integrate an understanding of the relationship between trauma, Substance Use and Mental Illness or Mental Health Issues.
Makes decisions based on evidence-based practice, reasoning, and clinical experience and in consultation with key stakeholders.
Possesses expert knowledge of Substance Use and Concurrent Disorders.
Possesses considerable knowledge and understanding of historical, cultural, spiritual, political and socio-economic issues that affect diverse populations and develops effective, inclusive and respectful working relationships with these individuals and groups.
Participates in developing, implementing or evaluating large or complex programs, or leads such efforts for smaller initiatives/programs.
Collaborates with colleagues in integrating knowledge with practice, other agencies and service providers to address challenges like barriers to service and gaps in the system.
Practices knowledge exchange principles in both written and verbal communication (simple, clear, direct, respectful, timely, evidence-based).

QUALIFICATIONS

Two year college diploma in Human Services Counseling, or Social Service Worker or university degree preferred with a related program that has a significant portion of its course content in addictions or concurrent disorders.
Experience working with clients going through withdrawal.
Extensive knowledge of Withdrawal Management Services, addictions and social service resources in the community.
Experience working in a cross-disciplinary team or in a medical environment dealing with addictions.
Experience and knowledge of coordinated access in addictions.
Crisis Intervention training.
CPR/First Aid
Excellent written and verbal communications skills.
Competencies in Word, Excel, and PowerPoint.
Ability to work flexible hours which includes days, evenings, 7 days a week.
