The UTSC Health & Wellness Centre provides health promotion, primary care and counselling services in an interprofessional shared care model. The team delivers student-focused care and support that is compassionate, culturally responsive and evidence-based and supports students in achieving their goals and potential by inspiring hope, promoting healthy choices and supporting the healing process. Reporting to the Director, Health & Wellness Centre, the Wellness Case Coordinator is the primary contact for students who present at the Health & Wellness Centre with complex health needs which require coordination of multiple support services. The Coordinator works to build and strengthen networks in the University and community to provide support for students experiencing issues of concern as a result of personal and/or academic situations.

The Coordinator serves as a resource to faculty, staff and students by responding quickly and effectively in the early stages to students who have complex issues, requiring skilled facilitation of appropriate referrals and providing ongoing case management where required. The Coordinator consults with and makes referral to University services such as the Community Safety Office, Campus Safety, Student Crisis Coordinators, AccessAbility Services and Academic Progress where necessary.

**Your responsibilities will include**:

- Providing case management support to students requiring the coordination of multiple services.
- Liaising with a health care team to facilitate collaboration on cases.
- Providing consultative advice to contacts regarding students in distress.
- Providing initial clinical support and/or counselling to distressed students in crisis and making referrals to appropriate resources.
- Fostering positive relationships with campus resources and community health services providers.
- Leading seminars and/or workshops.
- Following rules and procedural processes related to Freedom of Information and Protection of Privacy Act (FIPPA).
- Exercising extreme care to prevent serious physical or emotional injury or harm.

**Essential Qualifications**:

- Master's Degree in counselling/psychotherapy or social work or acceptable combination of equivalent experience. Must be in good standing with professional college.
- Minimum 5 years of relevant case management and counselling experience in a community mental health setting.
- Extensive experience with mental health and illness, behavioural issues, anger management and conflict resolution.
- Proven ability working with the young adult population and individuals with complex health and/or personal issues.
- Strong case management skills, with a demonstrated ability to be resourceful in their approach to supporting students.
- Ability to communicate effectively both verbally and in writing with a diverse student population.
- Understanding of professional boundaries and limitations and when to make appropriate referrals is required.
- Proficient with Microsoft Office (Word, Excel, Teams, Outlook and PowerPoint).
- Strong organizational skills, time management skills and initiative.
- Demonstrated commitment to working professional and collaboratively as a team player in an integrated multi-disciplinary environment.
- Must show evidence of a commitment to equity, diversity, inclusion and the promotion of a respectful and collegial learning and working environment.
- Knowledge and competency in maintaining confidentiality of client information according to FIPPA and PHIPA legislation.
- Strong knowledge of community mental health resources.
- Strong interpersonal skills and demonstrated ability to relate to students, staff and faculty in ways that build confidence and trust.
- Able to work in a highly diverse community with students from varied backgrounds and a variety of age groups, presenting with a range of difficulties and personal issues.
