To help fulfill its mission, LOFT is hiring a **Psychogeriatric Case Manager**.

**Job Summary**

As a Psychogeriatric Case Manager you will be part of a Mobile Support Team that provides

direct care, education and support to older adults and their care partners, who are

demonstrating responsive behaviors that may be associated with various types of dementia,

mental health, substance use or other neurological conditions living within the community

within a specified geographical boundary. You will complete a comprehensive behavioral

assessment and an evidence-based Behavioral Support Plan. You will assist in the creation

and coordination of a support network through establishing linkages with other community

agencies and practitioners to coordinate care for the clients. In addition, you will provide

direct service to support the client, family members and/or care-givers to mitigate

escalating responsive behaviors. You will also liaise, collaborate and communicate with all

members of the Behavioral Support Services team and other partners within the circle of

care.

The Behavioural Support Services - Mobile Support Team provides direct care, education and support to older adults and their care partners, who are demonstrating responsive behaviours/personal expressions that may be associated with various types of dementia, mental health, substance use or other neurological conditions living within the community within North Toronto, York Region and South Simcoe. The team completes a comprehensive behaviour support assessment and creates a person-centered Behavioural Support Plan. Transition support is provided to those individuals who a transitioning to another care destination or to another care team. Serving over 900 individuals per year, the Behaviour Support Service Mobile Support Team delivers service in the individuals own home and community.

**What You Will Do**
- Assists persons served in the development and implementation of a Behavioural Support Plan tailored to the needs of the person served and issues designed to improve quality of life and assist in the mitigation of responsive behaviours. Identify and refer to resources specific to those needs and issues;
- Act as a liaison and advocate for the person served among those involved with the individual’s care including family, other care-givers, medical and other multi-disciplinary providers;
- Complete PIECES/Functional Assessment and provide timely and accurate record keeping and reports as required;
- Provide support, on-going training and consultation to colleagues, family members and/or other care-givers to assist in mitigating escalating responsive behaviours;
- Provide supportive counselling or referral and linkages to health care, programs or service providers;
- Provide crisis management/intervention and assist in the development of crisis plans for persons served;
- Develop respectful and trusting relationships with persons served using Recovery and Psychosocial Rehabilitation principles;
- Maintain strict confidentiality and follow all other required policies and procedures;
- Consistently interact with persons served, staff, family members, other community agencies and partners in a professional manner that reflects favourably on the organization and the Behavioural Supports Ontario Project;
- Effectively participate as a member of the Behaviour Support Resource Team; attend staff/team meetings, care conferences for the person served, other multi-disciplinary meetings, committee meetings etc as required;
- Attend and actively participate in all required training;
- Liaise, collaborate and coordinate with other professionals, service providers and partners within the Behavioural Support Services;
- Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption pursuant to the Ontario Human Rights Code in accordance with LOFT’s Communicable Diseases policy and program guidelines
- It is an expectation of the position that most of the work will be conducted in hospitals and compliance with their occupational health polices and protocols will be required (associated costs will be reimbursed)
