Position: CLINICAL COORDINATOR (NURSING)
- Adult Congenital Heart Disease(ACHD) Transition Nurse

Reports to: MANAGER

Hours: 37.5 HOURS PER WEEK; Shifts: DAYS

**Salary**: $41.75 - $52.19 PER HOUR

Status: TEMPORARY FULL-TIME

As a member of Peter Munk Cardiac Centre team, the Clinical Coordinator

OTHER JOB RESPONSIBILITIES:

- Collaborate with the SickKids Transition Team to organize and facilitate bi-annual Transition Family Events
- Support and promote the hospital’s purpose, values and principles with all patients, families and colleaguesMaintain consistent patient safety and quality management responsibilities that sustain a culture aligned with UHN’s Purpose, Values and Principles as well Caring Safely initiatives (HACs); including safety behaviors and the use of error prevention tools
- Maintain collaboration with unit and/or department operational leadership
- Participate in or lead quality and safety initiatives and/or organizational committees based on clinical expertise
- Act as a patient advocate by identifying system issues that could adversely affect patient care, along with providing potential solutions
- Work in compliance of the Occupational Health & Safety Act and its regulations, reporting hazards, deficiencies and contraventions of the Act, in a timely manner
- Adhere to hospital policy (lateness, civility, mandatory e-learning); workplace safety
- Establish metrics and conduct data collection in order to understand RN role impact and for programmatic evaluation
- Design, direct and evaluate quality improvement methodologies to promote safe, timely, effective, efficient, equitable and patient centered care
- Translate research findings into clinical practice for improved patient outcomes and to support nursing/inter-professional practice

QUALIFICATIONS:

- Current registration with the College of Nurses of Ontario
- Bachelor of Science in Nursing
- Current BCLS Certification
Minimum of five (5) years clinical nursing experience, preferably in Adult Congenital heart disease
- Ambulatory experience preferred
- Experience in change management & quality improvement
- Strong critical thinking and problem solving skills
- Strong organizational and time management skills
- Excellent interpersonal and customer service skills
- Excellent verbal and written communication skills
- Demonstrated ability to cultivate and build relationships
- Strong track record of high quality work and engagement is essential
- Able to adapt to a constantly changing environment that requires ongoing revaluation of priorities
- Demonstrated ability to use good judgment in assessing difficult situations
- Able to work independently & collaboratively within a group setting
- Proven track record of taking initiative
- Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code.
