We currently have a** Temporary Part-time (0.4 FTE, until approx. March 2024) **opportunity for a **Project Lead - Early Warning Score Initiative **on the **Quality and Patient Safety** at our **Bayview **campus.

**Hours of Work**:
Weekdays: Days; 8 hrs, As per schedule

**General Description**

Sunnybrook Health Sciences Centre has a temporary, part time (0.3FTE - 0.4FTE) vacancy for a Clinical Project Lead within the Department of Quality & Patient Safety, focused on the implementation and evaluation of an Early Warning Scoring System to support teams in recognizing early signs of clinical deterioration and improving overall Escalation of Care. The Clinical Project Lead will work in partnership with the Early Warning Scores project manager (from the Project Management Office) and the Early Warning Scores Advisory Committee to design, test and implement this system. This role will report to the Manager of Quality & Patient Safety.

The successful individual will play an essential role in all aspects of this hospital wide patient safety initiative, including the development of policies, implementation plans, evaluation metrics, interprofessional education and more. This individual will collaborate with teams across programs integrating strong critical thinking and problem solving, and ideally has a strong clinical background in acute care or critical care.

There will additional opportunities to provide leadership for emerging strategies that advance the delivery of person centred care and best practices.

**Summary of Duties**
- Co-lead the implementation and evaluation of the Early Warning Scores Project
- Lead the Early Warning Scores Advisory Group in the co-design of the processes, policies, workflow and implementation plan
- Collaborate with organizational and physician leaders to ensure support for and awareness of the EWS
- System and Sunnybrook’s Escalation of Care patient safety goals.
- Lead the development of education material and communication strategies necessary to support the uptake of the EWS System
- Guide clinical teams in preparation for, support and adopt actions required
- In collaboration with the Project Manager (from the Project Management Office), create and maintain detailed project plans to monitor progress
- Identify project risks and work with the Project Manager, Advisory Group and Project Sponsor to mitigate and manage risks
- Analyze implementation results, evaluate options, and provide recommendations on next steps
- Lead meetings with stakeholders and coordinating internal resources for project implementation and communication
- Other duties as required

**Qualifications/Skills**
- Regulated Health Professional with current Regulatory College Registration in good standing required (nursing or respiratory therapy preferred due to nature of the project)
- Minimum 5 years of clinical experience in an inpatient setting required
- Experience with patient assessment and escalation of care required
- Project Management training or experience preferred
- Strong problem-solving and risk management skills
- Demonstrated successful track record with change management and leadership including effective implementation, evaluation and supporting / sustaining improvements.
- Excellent interpersonal and communication skills (oral, written and comprehension) working collaboratively within the inter-professional team.
- Proven organizational, time management, meeting facilitation and relationship-building skills
- Experience with MS office required
- Experience with literature searches & critical appraisal of published evidence required
