Hours: 18.5 hours per week or 37.5 hours bi-weekly; Monday-Friday (Evening Shifts) and/or Weekend (Variable Shifts)
Status: Permanent Part-Time

POSITION:
As an integral member of the **Joint Department of Medical Imaging**, the responsibilities of the **Clinical Intake Coordinator, Lung Cancer Screening** include: utilizing clinical knowledge/experience to serve as an entry point for the timely access to appropriate clinical assessment and treatment services; obtaining and maintaining current knowledge of internal/external services to communicate timely information; collaborating with representatives from all services to plan, develop, implement and evaluate a systems approach to client care; identifying gaps in service and participating in addressing such, internally and externally; playing a collaborative role with respect to the management of resource utilization; evaluating the effectiveness of the “role” to facilitate decision-making; perform cross-functional and/or other duties consistent with the job-classification, as assigned or requested

QUALIFICATIONS:

- At minimum, completion of a Bachelor’s degree in Health Sciences or recognized equivalent required
- Practical and related experience in direct clinical care, including intake functions/triaging as well as community relations and service development
- Previous experience in patient counselling
- Demonstrated ability to work independently and collaboratively as part of a multidisciplinary team
- Client service oriented and ability to work with the target population
- Demonstrated knowledge of clinical pathways and Continuous Quality Improvement (CQI)
- Knowledge of smoking cessation counselling techniques is an asset
- Ability to make quick assessment
- Proven skills as a problem recognition and problem solving
- Exercise initiative and good judgment with ability to multi-task
- Compliance with confidentiality requirements
- Effective organizational, interpersonal, and communication skills
- Knowledge of applicable legislative, UHN and/or departmental policies
- Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards
- Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code
