New Directions is a social service agency offering a wide range of resources and services that foster people’s hopes and dreams in their communities.
- Shift Staffed Homes **(SSH)** is one of the many services areas within New Directions, providing 24/7 support to over 65 individuals with intellectual/developmental disabilities who may experience complex health issues. The community-based service includes 20+ community homes located in Winnipeg and Springfield, supported by over 200 employees. SSH strives to promote a sense of belonging and to provide opportunities for individuals to have meaningful lives.
- The **Clinical Support Navigator** will be responsible for the development and implementation of enhanced clinical services and supports for the individuals supported by SSH to ensure a high quality of life and the ability to age in place. A focus of the position will be advocacy for individuals within complex systems, so they have undisrupted access to health, social and wellness services to achieve specific person-centered outcomes. The Clinical Support Navigator is responsible for completing relevant assessments as required to support their recommendations for service and programming.

This position will also be relied upon to provide person specific training to the SSH team and oversee clinical aspects of individual support plans, including behavioural and crisis management services. It is integral for the Clinical Support Navigator to work in cooperation with the SSH leadership team, collaborate with professionals from different disciplines and external systems, while maintaining strong relationships with the individuals receiving supports and their families.

**The successful applicant will have, as a minimum**:

- Master's degree in clinical field (e.g., MOT, MPsych, MSW, MMFT) or an equivalent advanced degree/certification in a clinical discipline.
- Be certified to practice in their discipline within the Province of Manitoba, by being accredited by a Provincial or National licensing body associated with the clinical discipline and in good standing,
- Clinical experience working with adults with intellectual disabilities and mental health challenges, with a trauma-informed understanding of behaviours and adaptive coping responses,
- Experience with care plan development and implementation, where the individual’s well-being is the focus,
- Demonstrated knowledge and ability to work with individuals with intellectual and developmental disabilities, and/or mental health challenges,
- Proven ability to provide effective leadership within a multi-disciplinary team environment, offering respect, professionalism, and cultural awareness,
- An understanding of person-centered frameworks, including a case management model of service, and a drive to ensure supports are implemented,
- Proven ability to foster and maintain effective working relationships with internal and external contacts,
- Clear and concise verbal and written communication skills and experience with professional communication with funding agencies,
- Comfort in the regular use of Microsoft Office suite of products,
- Valid Driver’s license and access to a safe, reliable vehicle for work purposes. Travel will include the city of Winnipeg, and may include some rural Manitoba areas
- Fluency in American Sign Language is considered an asset.
